COMPOSITION

1 tablet contains:

Vildagliptin — 50 mg. 

INDICATIONS FOR USE 

Type 2 diabetes mellitus:

    • as monotherapy in combination with diet and exercise; 
    • in combination with metformin as initial pharmacological therapy when diet and exercise alone do not provide adequate glycemic control; 
    • as part of dual combination therapy with metformin, a sulfonylurea, a thiazolidinedione, or insulin when diet, exercise, and monotherapy with these agents are insufficient; 
    • as part of triple combination therapy with a sulfonylurea and metformin in patients previously treated with a sulfonylurea and metformin, who have not achieved adequate glycemic control despite diet and exercise; 
    • as part of triple combination therapy with insulin and metformin in patients previously treated with insulin and metformin, who have not achieved adequate glycemic control despite diet and exercise. 

DOSAGE AND ADMINISTRATION

The product is administered orally, with or without food.

The dosage regimen should be individualized according to efficacy and tolerability.

The recommended dose as monotherapy or as part of dual combination therapy with metformin, a thiazolidinedione, or insulin (with or without metformin) is 50 mg or 100 mg daily. In patients with more severe type 2 diabetes mellitus receiving insulin therapy, a dose of 100 mg daily is recommended.

The recommended dose as part of triple combination therapy (vildagliptin + sulfonylurea + metformin) is 100 mg daily.

A dose of 50 mg should be taken once daily in the morning. A dose of 100 mg should be administered as 50 mg twice daily, in the morning and evening.

If a dose is missed, it should be taken as soon as possible. The recommended daily dose should not be exceeded.

When used as part of dual combination therapy with a sulfonylurea, the recommended dose is 50mg once daily in the morning.

When combined with sulfonylureas, the efficacy of 100 mg/day was comparable to that of 50mg/day.

If adequate glycemic control is not achieved with the maximum recommended daily dose of 100 mg, additional antidiabetic agents, including metformin, sulfonylureas, thiazolidinediones, or insulin, may be added.

PHARMACOLOGICAL PROPERTIES

Pharmacodynamics

Vildagliptin is a member of the class of pancreatic islet stimulators and a selective inhibitor of the enzyme dipeptidyl peptidase-4 (DPP-4). Rapid and complete inhibition of DPP-4 activity (>90%) results in increased basal and meal-stimulated secretion of glucagon-like peptide-1 (GLP-1) and glucose-dependent insulinotropic polypeptide (GIP) from the intestine into the systemic circulation throughout the day.

By increasing GLP-1 and GIP concentrations, vildagliptin enhances the sensitivity of pancreatic β-cells to glucose, thereby improving glucose-dependent insulin secretion.

CONTRAINDICATIONS

    • hypersensitivity to vildagliptin or any other component of the product; 
    • hereditary galactose intolerance, lactase deficiency, or glucose-galactose malabsorption. 

The efficacy and safety of the product in children under 18 years of age have not been established.

ADVERSE REACTIONS

When the product was used as monotherapy or in combination with other medicinal products, most adverse reactions were mild, transient, and did not require discontinuation of therapy.

DRUG INTERACTIONS

Vildagliptin has a low potential for drug interactions. It is neither a substrate, inhibitor, nor inducer of the major cytochrome P450 enzymes. No clinically significant interactions have been observed when administered concomitantly with metformin, glibenclamide, pioglitazone, amlodipine, digoxin, warfarin, and other commonly used medications.

Concomitant use with ACE inhibitors may increase the risk of angioedema.

SPECIAL PRECAUTIONS

Vildagliptin is not a substitute for insulin and should not be used in patients with type 1 diabetes mellitus or for the treatment of diabetic ketoacidosis.

Liver function tests should be performed before initiation of treatment, every 3 months during the first year of therapy, and periodically thereafter. If elevated hepatic transaminase levels are detected, the patient should be re-evaluated. Treatment should be discontinued if abnormalities persist.

Patients should be advised to seek medical attention immediately if symptoms suggestive of hepatic dysfunction occur, including nausea, vomiting, loss of appetite, jaundice, dark urine, or pain in the right upper abdomen.

If angioedema develops, treatment with the product should be discontinued.
