COMPOSITION

Each tablet contains:

Valacyclovir -  500 mg. 

INDICATIONS FOR USE 

·treatment of herpes zoster; 

·treatment and prevention of recurrence of skin and mucosal infections caused by herpes simplex virus (including initial and recurrent genital herpes); 

·treatment of labial herpes; 

·reduction of the risk of genital herpes transmission to an uninfected partner when used as suppressive therapy in combination with safe sex practices; 

·  prevention of cytomegalovirus infection in organ transplantation (reduces the severity of acute graft rejection in kidney transplant patients, as well as the development of opportunistic and other viral infections caused by Herpes simplex and Varicella zoster viruses) in adults and children over 12 years of age

DOSAGE AND ADMINISTRATION

Uviromed is administered orally.

Adults:

Herpes zoster: 1000 mg three times daily for 7 days.

Herpes simplex: 500 mg twice daily. For recurrences, treatment duration is 3 or 5 days. In the first severe episode, treatment may be extended up to 10 days. For recurrences, therapy is ideally started during the prodromal phase or at the first signs (tingling, itching, burning).

For labial herpes: 2 g twice within 1 day; the second dose should be taken about 12 hours after the first (but not earlier than 6 hours). This regimen should not be used for more than 1 day, as no additional clinical benefit has been demonstrated.

Prevention of recurrent herpes simplex infections:

    • immunocompetent patients: 500 mg once daily; 
    • frequent recurrences (10 or more per year): 250 mg twice daily; 
    • immunocompromised patients: 500 mg twice daily. Duration: 4–12 months. 

Prevention of genital herpes transmission to an uninfected partner: for infected heterosexual adults with preserved immunity and up to 9 outbreaks per year — 500 mg once daily for 1 year or longer during regular sexual activity. With irregular intercourse, treatment should be started 3 days before expected sexual contact. Data for other populations are not available.

Prevention of cytomegalovirus infection: adults and adolescents over 12 years — 2 g four times daily, started as soon as possible after transplantation. Duration: 90 days; longer in high-risk patients. Dose reduction is recommended in significant renal impairment.

PHARMACOLOGICAL PROPERTIES

Uviromed is an antiviral drug. In the human body, valaciclovir is rapidly and completely converted into aciclovir and L-valine by the enzyme valaciclovir hydrolase.

In vitro, aciclovir has specific inhibitory activity against Herpes simplex viruses types 1 and 2, Varicella zoster, Epstein–Barr virus, cytomegalovirus (CMV), and human herpesvirus type 6.

Aciclovir inhibits viral DNA synthesis after phosphorylation and conversion into its active form, aciclovir triphosphate.

The initial phosphorylation step is carried out by virus-specific enzymes. For Herpes simplex, Varicella zoster, and Epstein–Barr virus, this enzyme is viral thymidine kinase present in infected cells.

Partial selectivity of phosphorylation is also present in CMV and is mediated by the UL97 phosphotransferase gene product. Activation of aciclovir by virus-specific enzymes largely explains its selectivity.

CONTRAINDICATIONS

    • hypersensitivity to valaciclovir, aciclovir, or any of the excipients; 
    • clinically significant HIV infection with CD4+ lymphocyte count below 100/µL; 
    • bone marrow transplantation; 
    • kidney transplantation; 
    • children (under 12 years for CMV indications, under 18 years for other indications). 

With caution: 

hepatic insufficiency (especially at high doses), renal insufficiency, pregnancy, and lactation.

ADVERSE REACTIONS

The frequency categories of adverse effects used below are defined as follows: common (≥ 1/100 to < 1/10), uncommon (≥ 1/1000 to < 1/100), rare (≥ 1/10000 to < 1/1000), very rare (< 1/10000), and not known — frequency cannot be estimated from the available data.

The most common adverse reactions with valaciclovir are headache and nausea. More serious adverse reactions include thrombotic thrombocytopenic purpura/hemolytic uremic syndrome, acute renal failure, and neurological disorders.

DRUG INTERACTIONS

No clinically significant interactions have been identified.

Cimetidine and probenecid, after administration of 1 g valaciclovir, increase the AUC of aciclovir by reducing its renal clearance (however, no dose adjustment of valaciclovir is required due to the wide therapeutic index of aciclovir).

Caution is required when valaciclovir is used at high doses (4 g/day) with drugs that compete with aciclovir for elimination pathways (aciclovir is excreted unchanged in urine via active tubular secretion), as there is a potential risk of increased plasma concentrations of one or both drugs or their metabolites.

Co-administration of aciclovir with mycophenolate mofetil has been shown to increase the AUC of aciclovir and the inactive metabolite of mycophenolate mofetil.

Caution is also required when high-dose valaciclovir (≥4 g/day) is combined with drugs affecting renal function (e.g., ciclosporin, tacrolimus).

Careful use is required when valaciclovir is combined with nephrotoxic drugs, especially in patients with impaired renal function, with regular monitoring of renal function. This includes concomitant use of aminoglycosides, platinum compounds, iodinated contrast media, methotrexate, pentamidine, foscarnet, ciclosporin, and tacrolimus.

SPECIAL PRECAUTIONS

Patients at risk of dehydration, especially elderly patients, should maintain adequate hydration during treatment with Uviromed. Patients with renal impairment have an increased risk of developing neurological complications.

Aciclovir is eliminated from the body primarily via renal clearance; therefore, in patients with renal impairment, the dose of valaciclovir should be reduced.

No dose adjustment is required in elderly patients, except in cases of significant renal impairment. Adequate fluid and electrolyte balance should be maintained.
