About the Product
COMPOSITION
One tablet contains:
Nebivolol 5 mg.
INDICATIONS FOR USE
·arterial hypertension;
·ischemic heart disease: prevention of stable angina attacks;
· chronic heart failure (as part of combination therapy).
DOSAGE AND ADMINISTRATION
Nebiworld tablets are taken orally once daily, preferably at the same time each day, regardless of meals, with a sufficient amount of liquid.
The average daily dose for the treatment of arterial hypertension and ischemic heart disease is 2.5–5 mg of nebivolol. The optimal effect develops after 1–2 weeks of treatment, and in some cases after 4 weeks. If after 4 weeks of therapy with Nebiworld blood pressure control is insufficient, the dose may be increased over 2 weeks to 40 mg per day. Nebiworld can be used as monotherapy or in combination with other antihypertensive agents.
DOSAGE FORM
Tablets No. 28.
PHARMACOLOGICAL PROPERTIES
Pharmacodynamics
Nebiworld is a cardioselective β1-adrenergic blocker. It has antihypertensive, antianginal, and antiarrhythmic effects. Nebivolol is a racemic mixture of D- and L-enantiomers. The D-enantiomer provides selective competitive β1-adrenoceptor blockade, while the L-enantiomer induces vasodilation via interaction with the nitric oxide/L-arginine pathway.
At therapeutic doses, nebivolol has no intrinsic sympathomimetic activity, no significant membrane-stabilizing effect, and does not affect α-adrenergic receptors.
Single and repeated administration reduces heart rate (HR) and lowers blood pressure (BP) at rest and during exercise in both normotensive and hypertensive patients. The antihypertensive effect is maintained during long-term treatment.
The antihypertensive effect is also associated with a reduction in renin-angiotensin system activity (without a direct correlation with plasma renin activity).
Pharmacokinetics
After oral administration, both enantiomers of nebivolol are rapidly absorbed from the gastrointestinal tract. Food does not affect absorption, so the drug may be taken regardless of meals.
Bioavailability is approximately 12% in fast metabolizers (first-pass effect) and nearly complete in slow metabolizers.
Plasma protein binding:
- D-nebivolol: 98.1%
- L-nebivolol: 97.9%
Nebivolol is metabolized via aromatic and alicyclic hydroxylation and partial N-dealkylation. Hydroxy- and amino-derivatives are conjugated with glucuronic acid and excreted as O- and N-glucuronides.
Half-life:
- fast metabolizers: nebivolol 10 h, hydroxymetabolites 24 h
- slow metabolizers: nebivolol 30–50 h, hydroxymetabolites 48 h
Excretion: 38% via kidneys and 48% via intestines. Less than 0.5% is excreted unchanged in urine.
CONTRAINDICATIONS
- hypersensitivity to nebivolol or any component of the drug;
- decompensated heart failure;
- severe arterial hypotension (systolic BP < 90 mmHg);
- sick sinus syndrome, including sinoatrial block;
- severe hepatic impairment;
- bradycardia (HR < 60 bpm);
- second- and third-degree atrioventricular block (without a pacemaker);
- pheochromocytoma;
- metabolic acidosis;
- cardiogenic shock;
- bronchospasm and history of bronchial asthma;
- severe peripheral arterial occlusive disease (intermittent claudication, Raynaud’s syndrome);
- myasthenia gravis;
- depression.
ADVERSE REACTIONS
Headache, dizziness, increased fatigue, weakness.
DRUG INTERACTIONS
Concomitant use of nebivolol with nonsteroidal anti-inflammatory drugs does not reduce its antihypertensive effect.
When β-blockers are used together with slow calcium channel blockers (verapamil and diltiazem), there is an increased negative effect on myocardial contractility and atrioventricular conduction.
Intravenous administration of verapamil is contraindicated during treatment with nebivolol.
SPECIAL PRECAUTIONS
In patients with angina, treatment should be discontinued gradually over 1–2 weeks. In stable angina, the dose should maintain a resting heart rate of 55–60 bpm and not exceed 110 bpm during exercise.
In patients with diabetes mellitus, Nebivorld does not affect plasma glucose concentration, but like all β-blockers, it may mask signs of hypoglycemia.
