COMPOSITION

One tablet contains:

Active substance:

    • levofloxacin — 500 mg. 

INDICATIONS FOR USE 

Infectious and inflammatory diseases caused by microorganisms sensitive to the drug:

    • ENT infections: acute sinusitis; 
    • lower respiratory tract infections: acute exacerbation of chronic bronchitis, community-acquired pneumonia; 
    • complicated kidney and urinary tract infections, including pyelonephritis; 
    • uncomplicated urinary tract infections; 
    • chronic prostatitis; 
    • skin and soft tissue infections. 

DOSAGE AND ADMINISTRATION

Levoximed is taken orally once daily, regardless of meals. Tablets should be swallowed whole with sufficient liquid and not chewed.

Acute sinusitis: 500 mg once daily for 10–14 days.

 Acute exacerbation of chronic bronchitis: 500 mg once daily for 7 days.

 Community-acquired pneumonia: 500 mg once daily for 7–14 days.

 Complicated urinary tract infections and acute pyelonephritis: 500 mg once daily for 7-10 days.

 Uncomplicated urinary tract infections: 250 mg once daily for 3 days.

 Chronic prostatitis: 500 mg once daily for 28 days.

 Uncomplicated skin and soft tissue infections: 500 mg once daily for 7–10 days.

Patients with renal impairment require dose adjustment depending on creatinine clearance.

PHARMACOLOGICAL PROPERTIES

Levoximed is a broad-spectrum antimicrobial agent belonging to the fluoroquinolone group. Levofloxacin, the active substance of the medicinal product, is the optically active levorotatory isomer of ofloxacin — L-ofloxacin. Levofloxacin inhibits DNA gyrase (topoisomerase II) and topoisomerase IV, disrupts the re-ligation of DNA breaks, inhibits DNA synthesis, and induces profound morphological changes in the cytoplasm, cell wall, and membranes of bacterial cells.

Levofloxacin is active against most strains of microorganisms both in vitro and in vivo:

Aerobic Gram-positive microorganisms: Corynebacterium diphtheriae, Enterococcus spp. (including Enterococcus faecalis), Listeria monocytogenes, Staphylococcus spp., Staphylococcus aureus, Staphylococcus epidermidis, Streptococcus spp. groups C and G (including Streptococcus agalactiae, Streptococcus pneumoniae, Streptococcus pyogenes, Streptococcus viridans);

Aerobic Gram-negative microorganisms: Acinetobacter baumannii, Acinetobacter spp., Citrobacter freundii, Eikenella corrodens, Enterobacter aerogenes, Enterobacter agglomerans, Enterobacter spp. (including Enterobacter cloacae), Escherichia coli, Gardnerella vaginalis, Haemophilus ducreyi, Haemophilus influenzae, Haemophilus parainfluenzae, Helicobacter pylori, Klebsiella spp. (including Klebsiella oxytoca, Klebsiella pneumoniae), Moraxella catarrhalis, Morganella morganii, Neisseria gonorrhoeae, Neisseria meningitidis, Pasteurella spp. (including Pasteurella canis, Pasteurella dagmatis, Pasteurella multocida), Proteus mirabilis, Proteus vulgaris, Providencia spp. (including Providencia rettgeri, Providencia stuartii), Pseudomonas spp. (including Pseudomonas aeruginosa), Salmonella spp., Serratia spp. (Serratia marcescens);

Anaerobic microorganisms: Bacteroides fragilis, Bifidobacterium spp., Clostridium perfringens, Fusobacterium spp., Peptostreptococcus spp., Propionibacterium spp., Veillonella spp.;

Other microorganisms: Bartonella spp., Chlamydia pneumoniae, Chlamydia psittaci, Chlamydia trachomatis, Legionella pneumophila, Legionella spp., Mycobacterium spp. (including Mycobacterium leprae, Mycobacterium tuberculosis), Mycoplasma hominis, Mycoplasma pneumoniae, Rickettsia spp., Ureaplasma urealyticum.

CONTRAINDICATIONS

    • hypersensitivity to the components of the medicinal product or to other quinolones; 
    • epilepsy; 
    • history of tendon disorders associated with quinolone use; 
    • pregnancy and breastfeeding period; 
    • children and adolescents under 18 years of age. 

Precautions

The medicinal product should be used with caution in elderly patients due to a higher likelihood of concomitant reduced renal function, as well as in patients with glucose-6-phosphate dehydrogenase deficiency.

ADVERSE REACTIONS

Gastrointestinal system:

 common — nausea, diarrhea; uncommon — loss of appetite, vomiting, abdominal pain, dyspepsia; rare — bloody diarrhea.

Metabolism:

 very rare — hypoglycemia; in isolated cases — exacerbation of existing porphyria.

Central and peripheral nervous system:

 uncommon — headache, dizziness, stiffness, drowsiness; rare — paresthesia in the hands, tremor, anxiety, fear, seizures, confusion; very rare — visual and auditory disturbances, taste and smell disorders, decreased tactile sensitivity, psychotic reactions (hallucinations, depression), motor disturbances (including gait disorders).

Cardiovascular system:

 rare — tachycardia, hypotension; very rare — vascular collapse.

Musculoskeletal system:

 rare — tendon disorders, joint and muscle pain.

Urinary system:

 rare — increased serum creatinine; very rare — renal impairment up to acute renal failure.

Hematopoietic system:

 uncommon — eosinophilia, leukopenia; rare — neutropenia, thrombocytopenia; very rare — severe agranulocytosis accompanied by persistent or recurrent fever, tonsillitis, and marked deterioration of general condition with possible severe infections.

Allergic reactions:

 uncommon — pruritus and skin redness; rare — anaphylactic and anaphylactoid reactions (urticaria, bronchospasm, severe dyspnea, and in rare cases facial and laryngeal edema).

Dermatological reactions:

 rare — photosensitivity.

DRUG INTERACTIONS

The effect of Levoximed is significantly reduced when administered concomitantly with sucralfate, as well as with antacid preparations containing magnesium or aluminium, and with iron salts.

Levofloxacin, like all quinolones, may enhance the ability of certain medicinal products, including non-steroidal anti-inflammatory drugs and theophylline, to lower the seizure threshold.

The renal clearance of levofloxacin is slightly reduced by cimetidine and probenecid; however, this effect is of no clinical significance.

When co-administered with vitamin K antagonists, monitoring of blood coagulation parameters is required.

Levofloxacin causes a slight increase in the elimination half-life of ciclosporin from plasma.

Concomitant use with glucocorticosteroids increases the risk of tendon rupture.

SPECIAL PRECAUTIONS

When prescribing Levoximed, caution should be exercised in:

    • elderly patients, due to a higher likelihood of concomitant reduced renal function and an increased risk of tendon rupture; 
    • patients with a history of brain lesions, including stroke or severe traumatic brain injury, due to increased seizure predisposition; 
    • patients with diabetes mellitus, as hypoglycaemia may occur; 
    • patients with impaired renal function; the medicinal product should be used with caution when administered concomitantly with drugs that inhibit tubular secretion.
