Each 10 ml contains:

Active substance:

    • Levocarnitine — 1.0 g 

INDICATIONS FOR USE 

·primary (genetic) carnitine deficiency; 

·secondary carnitine deficiency in patients receiving long-term hemodialysis and presenting with the following symptoms: 

    • severe persistent muscle cramps; 
    • skeletal muscle weakness; 
    • myopathies; 
    • loss of muscle mass; 
    • symptoms of arterial hypotension; 
    • anemia; 
    • cardiomyopathy; 
    • lack of response to treatment or the need for high doses of erythropoietin

DOSAGE AND ADMINISTRATION

This medication is intended for oral use. Before use, the solution should be diluted in a glass of water and taken 30 minutes before meals. Use a dosing syringe or measuring cup for dosing.

Primary and secondary carnitine deficiency

The recommended oral dose is 100 to 200 mg/kg of body weight per day, divided into 2-4 doses. For less severe conditions, the dose may be lower (50-100 mg/kg of body weight per day).

Children under 12 years of age (including newborns and infants)

The drug can be used in children from the first day of life, including premature babies. The solution is administered starting with a dose of 50 mg/kg of body weight per day. The usual dose for children is 50-100 mg/kg of body weight per day.

Adults and children over 12 years of age

For acute decompensation, the drug is prescribed at a dose of 50-100 mg/kg per day. If necessary, higher doses can be used; however, this is associated with an increased risk of adverse reactions.

PHARMACOLOGICAL PROPERTIES

Pharmacokinetics

Absorbed L-carnitine is transported to various systems and organs via the blood. Carnitine concentrations in tissues and serum depend on metabolic rate, carnitine biosynthesis, dietary factors, its transport into and out of tissues, metabolism, and elimination. Pharmacokinetic parameters increase significantly with dosage. Total bioavailability is approximately 10-16%. Available data indicate a relationship between peak plasma levocarnitine concentration and dose, plasma area under the curve (AUC), dose, and urinary accumulation. After oral administration, it is well absorbed from the gastrointestinal tract (80%). Peak concentrations are reached four hours after administration. From plasma, it penetrates the liver, myocardium, and skeletal muscle. It is excreted by the kidneys, primarily as acyl esters.

Pharmacodynamics

By releasing coenzyme A, levocarnitine enhances the entry of metabolites into the Krebs cycle. It also stimulates pyruvate dehydrogenase activity in skeletal muscles and promotes the oxidation of branched-chain amino acids.

The drug normalizes protein and fat metabolism, restores the blood alkaline reserve, inhibits ketoacid formation and anaerobic glycolysis, reduces lactacidosis, increases physical activity and exercise tolerance, and promotes efficient glycogen utilization while increasing glycogen stores in the liver.

CONTRAINDICATIONS

·hypersensitivity to levocarnitine or any of the excipients of the product; 

ADVERSE REACTIONS

·nausea, vomiting, abdominal pain, and diarrhea (during prolonged use of oral levocarnitine); 

·muscle weakness and seizures in patients with uremia; 

·allergic reactions (skin itching, rash, urticaria, angioedema, anaphylactic shock); 

·characteristic body odor. Dose reduction often reduces or eliminates the unpleasant body odor and gastrointestinal symptoms; 

DRUG INTERACTIONS

Administration of L-Viava to patients with diabetes mellitus receiving insulin or oral hypoglycemic agents may cause hypoglycemia due to increased glucose utilization. Therefore, plasma glucose levels should be monitored regularly during treatment to allow appropriate adjustment of hypoglycemic therapy.

Concomitant use:

    • glucocorticosteroids promote accumulation of levocarnitine in tissues (except the liver); 
    • lipoic acid and anabolic agents enhance the effect of levocarnitine; 
    • carnitine may increase serum free valproic acid levels and thereby potentiate its effects even at therapeutic doses; 
    • excessive doses of choline may reduce the activity of carnitine. 

Patients should inform their physician about any other medications being taken concurrently with L-Viava.

SPECIAL PRECAUTIONS

The product does not cause habituation or dependence, as L-carnitine is a natural component of the human body.

Tolerance should be monitored during the first week of treatment and after each dose increase.

L-Viava should be used with caution in patients with:

    • chronic heart failure; 
    • acute renal failure; 
    • edema associated with salt retention; 
    • those receiving corticosteroids or corticotropic hormonal preparations. 

Long-term treatment without additional potassium supplementation may lead to hypokalemia.

The product should be used cautiously in patients with a history of allergic reactions.

Since carnitine enhances glucose utilization, blood glucose levels should be regularly monitored in patients with diabetes mellitus.

Intravenous administration of high doses in patients with severe renal impairment or those undergoing dialysis for end-stage renal disease does not result in accumulation of potentially toxic metabolites such as trimethylamine (TMA) and trimethylamine N-oxide (TMAO), which are normally excreted in urine. Such accumulation may occur during prolonged oral administration of high doses.

Pregnancy and Lactation

L-Viava injection solution should be used during pregnancy and breastfeeding only when clearly necessary, after careful assessment of the expected benefit to the mother and the potential risk to the fetus or child.

Effects on Ability to Drive and Operate Machinery

Patients who experience muscle weakness after administration of L-Viava should refrain from driving vehicles and operating potentially hazardous machinery.
