    • Levocarnitine — 1.0 g  

INDICATIONS FOR USE 

·primary (genetic) carnitine deficiency; 

·secondary carnitine deficiency in patients receiving long-term hemodialysis and presenting with the following symptoms: 

    • severe persistent muscle cramps; 
    • skeletal muscle weakness; 
    • myopathies; 
    • loss of muscle mass; 
    • symptoms of arterial hypotension; 
    • anemia; 
    • cardiomyopathy; 
    • lack of response to treatment or the need for high doses of erythropoietin

DOSAGE AND ADMINISTRATION

L-Viava solution for injection is intended for slow intravenous administration (over 2–3 minutes).

L-Viava injection solution is intended for slow intravenous administration (over 2-3 minutes) or intravenous infusion. Before intravenous drip infusion, the drug is dissolved in 0.9% sodium chloride or Ringer's lactate solution to obtain a solution with a levocarnitine concentration of 0.5-8 mg/ml.

 The dose is calculated according to age and body weight.

Primary and secondary carnitine deficiency

The drug is administered slowly intravenously over 2–3 minutes. The daily dose may be divided into 3–4 administrations or given as an intravenous infusion at approximately 3 ml/min (not more than 60 drops/min). Before administration, the required dose should be diluted in 100–250 ml of isotonic sodium chloride solution or 5% glucose solution.

Children under 12 years (including newborns and infants)

The recommended dose is generally 100–200 mg/kg/day. If clinical and biochemical parameters do not improve, the dose may be temporarily increased. In acute metabolic decompensation, doses up to 400 mg/kg/day may be required.

Adults and children over 12 years

In cases of acute decompensation, the recommended dose is 50–100 mg/kg/day. Higher doses may be used if necessary, although the risk of adverse reactions increases.

Secondary carnitine deficiency in hemodialysis patients

The drug should be administered slowly intravenously (over 2–3 minutes) after each hemodialysis session.

    • Children under 12 years: 10–20 mg/kg/day. 
    • Children over 12 years: 1 g/day. 
    • Adults: 2 g/day. 

A dose of 2.5 g is recommended for patients undergoing hemodialysis for more than one year.

The duration of intravenous treatment should not exceed 3 months. The need for repeated treatment courses should be determined by regular monitoring of plasma levocarnitine levels.

PHARMACOLOGICAL PROPERTIES

Pharmacokinetics

After intravenous administration, levocarnitine readily penetrates all tissues. High concentrations are achieved in skeletal muscles and the myocardium. Maximum plasma concentration is maintained for up to 9 hours. It does not bind to plasma proteins or human albumin. Following parenteral administration, levocarnitine undergoes minimal metabolism.

It is excreted by the kidneys, mainly as acyl esters, in amounts directly proportional to plasma levocarnitine levels. The elimination half-life is 17.4 hours. Approximately 76% of the administered dose is excreted by the kidneys within 24 hours.

Pharmacodynamics

L-Viava is a medicinal product for the correction of metabolic disorders.

Levocarnitine is a naturally occurring substance related to B vitamins. It participates in metabolic processes by transporting fatty acids across cell membranes from the cytoplasm into mitochondria, where they undergo β-oxidation, producing large amounts of energy in the form of ATP.

By releasing coenzyme A, levocarnitine enhances the entry of metabolites into the Krebs cycle. It also stimulates pyruvate dehydrogenase activity in skeletal muscles and promotes the oxidation of branched-chain amino acids.

The drug normalizes protein and fat metabolism, restores the blood alkaline reserve, inhibits ketoacid formation and anaerobic glycolysis, reduces lactacidosis, increases physical activity and exercise tolerance, and promotes efficient glycogen utilization while increasing glycogen stores in the liver.

CONTRAINDICATIONS

·hypersensitivity to levocarnitine or any of the excipients of the product; 

·for the infusion solution: hypernatremia and hypervolemia; 

·  infusion solutions prepared using glucose as a diluent are contraindicated in patients with diabetes mellitus.

ADVERSE REACTIONS

·nausea, vomiting, abdominal pain, and diarrhea (during prolonged use of oral levocarnitine); 

·muscle weakness and seizures in patients with uremia; 

·allergic reactions (skin itching, rash, urticaria, angioedema, anaphylactic shock); 

·characteristic body odor. Dose reduction often reduces or eliminates the unpleasant body odor and gastrointestinal symptoms; 

·  rapid intravenous administration may cause pain along the vein, which usually resolves when the infusion rate is reduced.

DRUG INTERACTIONS

Administration of L-Viava injection solution to patients with diabetes mellitus receiving insulin or oral hypoglycemic agents may cause hypoglycemia due to increased glucose utilization. Therefore, plasma glucose levels should be monitored regularly during treatment to allow appropriate adjustment of hypoglycemic therapy.

Concomitant use:

    • glucocorticosteroids promote accumulation of levocarnitine in tissues (except the liver); 
    • lipoic acid and anabolic agents enhance the effect of levocarnitine; 
    • carnitine may increase serum free valproic acid levels and thereby potentiate its effects even at therapeutic doses; 
    • excessive doses of choline may reduce the activity of carnitine. 

Patients should inform their physician about any other medications being taken concurrently with L-Viava.

SPECIAL PRECAUTIONS

The product does not cause habituation or dependence, as L-carnitine is a natural component of the human body.

Tolerance should be monitored during the first week of treatment and after each dose increase.

L-Viava injection solution should be used with caution in patients with:

    • chronic heart failure; 
    • acute renal failure; 
    • edema associated with salt retention; 
    • those receiving corticosteroids or corticotropic hormonal preparations. 

Long-term treatment without additional potassium supplementation may lead to hypokalemia.

The product should be used cautiously in patients with a history of allergic reactions.

Since carnitine enhances glucose utilization, blood glucose levels should be regularly monitored in patients with diabetes mellitus.

Intravenous administration of high doses in patients with severe renal impairment or those undergoing dialysis for end-stage renal disease does not result in accumulation of potentially toxic metabolites such as trimethylamine (TMA) and trimethylamine N-oxide (TMAO), which are normally excreted in urine. Such accumulation may occur during prolonged oral administration of high doses.

Pregnancy and Lactation

L-Viava injection solution should be used during pregnancy and breastfeeding only when clearly necessary, after careful assessment of the expected benefit to the mother and the potential risk to the fetus or child.

Effects on Ability to Drive and Operate Machinery

Patients who experience muscle weakness after administration of L-Viava injection solution should refrain from driving vehicles and operating potentially hazardous machinery.
