COMPOSITION

One tablet contains:

Loxoprofen — 60 mg

INDICATIONS FOR USE 

The medicinal product is indicated for use in adults aged 18 years and older.

As an anti-inflammatory and analgesic agent in the following conditions:

    • rheumatoid arthritis; 
    • osteoarthritis; 
    • lumbago (lower back pain); 
    • scapulohumeral periarthritis (shoulder syndrome); 
    • cervicobrachial syndrome; 
    • dental pain (including after tooth extraction); 
    • post-traumatic and postoperative pain syndromes. 

As an antipyretic and analgesic agent in:

    • acute upper respiratory tract infections (including acute upper respiratory tract infections associated with acute bronchitis). 

DOSAGE AND ADMINISTRATION

Rheumatoid arthritis, osteoarthritis, lumbago (lower back pain), scapulohumeral periarthritis (shoulder syndrome), cervicobrachial syndrome, dental pain (including after extraction), post-traumatic and postoperative pain syndrome — as an anti-inflammatory and analgesic agent

The recommended dose is 60 mg (1 tablet) three times daily.

Depending on the severity of symptoms, patients under 65 years of age may be prescribed 120 mg (2 tablets) of loxoprofen as a single dose.

In this case, the dosing frequency is twice daily.

The maximum daily dose of loxoprofen should not exceed 180 mg (3 tablets).

Acute upper respiratory tract infections (including acute upper respiratory tract infection with acute bronchitis) — as an antipyretic and analgesic agent

The recommended dose is 60 mg (1 tablet).

The usual frequency of administration is 1–2 times daily.

When selecting the dose, the patient's age and severity of symptoms should be taken into account.

The maximum daily dose of loxoprofen should not exceed 180 mg (3 tablets).

PHARMACOLOGICAL PROPERTIES

Loxoprofen is a non-steroidal anti-inflammatory drug (NSAID) with pronounced anti-inflammatory, analgesic, and antipyretic effects.

Its mechanism of action is associated with the inhibition of cyclooxygenase enzymes (COX-1 and COX-2), resulting in reduced synthesis of prostaglandins — the main mediators of inflammation, pain, and fever.

After administration, loxoprofen is converted into an active metabolite that inhibits COX activity, thereby reducing inflammatory responses and pain.

Main effects of the drug:

    • reduction of inflammation; 
    • relief of pain; 
    • reduction of elevated body temperature. 

Loxoprofen has a strong analgesic effect and is used for inflammatory musculoskeletal disorders and various types of acute pain.

CONTRAINDICATIONS

·hypersensitivity to loxoprofen or any of the excipients; 

·active peptic ulcer of the stomach and duodenum; 

·inflammatory bowel diseases (Crohn’s disease, ulcerative colitis); 

·active erosive and ulcerative lesions of the gastrointestinal tract; 

·history of gastrointestinal bleeding or perforation, including those associated with previous use of NSAIDs; 

·active gastrointestinal bleeding; 

·other active bleeding (including suspected intracranial haemorrhage); 

·severe haematological disorders; 

·severe hepatic impairment; 

·severe renal impairment, progressive kidney disease, confirmed hyperkalaemia; 

·severe heart failure (NYHA class IV — New York Heart Association classification); 

·postoperative period after coronary artery bypass grafting; 

·  complete or incomplete combination of bronchial asthma, recurrent nasal and paranasal sinus polyposis, and intolerance to acetylsalicylic acid or other NSAIDs (including history); 

·pregnancy (after 20 weeks); 

·  breastfeeding period.

ADVERSE REACTIONS

Other adverse reactions that may occur during treatment with Scorolox are classified according to MedDRA system organ classes and frequency according to the WHO classification:

Blood and lymphatic system disorders

    • Frequency unknown: anaemia, leukopenia, thrombocytopenia.

    • Uncommon: somnolence. 
    • Rare: headache, dizziness, numbness. 

Cardiac disorders

    • Rare: palpitations. 

Vascular disorders

    • Rare: increased blood pressure. 

Gastrointestinal disorders

    • Uncommon: abdominal pain, stomach discomfort, anorexia, nausea, diarrhoea, constipation, heartburn, stomatitis, abdominal distension, dry mouth. 
    • Rare: vomiting. 
    • Frequency unknown: peptic ulcers*, ulcers of the small and/or large intestine*, dyspepsia. 

Hepatobiliary disorders

    • Uncommon: increased AST and ALT activity. 
    • Rare: increased alkaline phosphatase activity. 

Skin and subcutaneous tissue disorders

    • Uncommon: rash*, pruritus*. 
    • Frequency unknown: fever*, urticaria*. 

Renal and urinary disorders

    • Uncommon: proteinuria. 
    • Frequency unknown: haematuria, dysuria, decreased urine output. 

General disorders

    • Uncommon: oedema, facial flushing (feeling of heat). 
    • Frequency unknown: chest pain, general malaise, sweating.

DRUG INTERACTIONS

Loxoprofen, like other NSAIDs, may interact with the following medicines:

    • Anticoagulants (e.g., warfarin) — may enhance anticoagulant effects and increase the risk of bleeding. 
    • Antiplatelet agents (e.g., aspirin, clopidogrel) — increased risk of gastrointestinal bleeding. 
    • Other NSAIDs and corticosteroids — increased risk of gastrointestinal ulceration and bleeding. 
    • Selective serotonin reuptake inhibitors (SSRIs) — may increase the risk of gastrointestinal bleeding. 
    • Diuretics — may reduce their effectiveness and increase the risk of renal impairment. 
    • ACE inhibitors and angiotensin II receptor blockers — may reduce antihypertensive effects and worsen renal function. 
    • Methotrexate — may increase methotrexate blood levels and toxicity. 
    • Lithium — may increase plasma lithium concentration and risk of toxicity. 
    • Cyclosporine and tacrolimus — increased risk of nephrotoxicity. 
    • Potassium-containing medicines — may increase blood potassium levels (hyperkalaemia).

SPECIAL PRECAUTIONS

The use of NSAIDs, including loxoprofen, is contraindicated from the 20th week of pregnancy due to the possible risk of oligohydramnios and/or renal impairment in newborns (neonatal renal dysfunction).

The use of loxoprofen during early pregnancy (before 20 weeks) may be considered only when the expected benefit outweighs the potential risk to the fetus.

If treatment is necessary, the lowest effective dose should be used, and the volume of amniotic fluid should be carefully monitored when appropriate.

Lactation

Loxoprofen passes into the breast milk of laboratory animals. A decision should be made either to discontinue breastfeeding or to discontinue treatment with the medicinal product.

Fertility

There are no data on the effect of loxoprofen on human fertility.

Available evidence indicates that prolonged use of NSAIDs may cause reversible impairment of fertility in women.
