About the Product

COMPOSITION

1 ampoule contains:

Active substance: iron (III) hydroxide sucrose complex, equivalent to elemental iron — 100 mg.

Excipient: water for injections. 

INDICATIONS FOR USE 

Gembag-S is indicated in iron deficiency conditions such as:

    • severe posthemorrhagic anemia; 
    • iron deficiency anemia of various origins in adults and children when oral iron therapy is ineffective or not feasible (including in patients with gastrointestinal diseases and malabsorption syndrome).
    • The drug should be used only in iron deficiency states confirmed by appropriate laboratory tests. 

DOSAGE AND ADMINISTRATION

This medication is intended for intravenous administration only, administered by very slow injection or drip infusion. It should not be administered subcutaneously or intramuscularly.

This medication should not be mixed with other medicinal products, with the exception of sterile 0.9% sodium chloride solution, which is used for dilution.

DOSAGE FORM

Solution for injection in yellow glass ampoules, 5 ml each.

5 ampoules in a blister pack.  One blister pack together with a package insert is placed in a cardboard box.

PHARMACOLOGICAL PROPERTIES

PHARMACODYNAMICS

Antianemic agent. Iron transport in plasma is carried out by the γ-globulin transferrin synthesized in the liver. Ferric iron (Fe³⁺) participates in heme formation, leading to an increase in hemoglobin levels. Administration of the drug results in gradual regression of clinical (weakness, fatigue, dizziness, tachycardia, dry skin) and laboratory signs of iron deficiency. The iron in the preparation is a component of hemoglobin and stimulates erythropoiesis in iron deficiency states. It is present as a sucrose iron (III) hydroxide complex consisting of a ferric core surrounded by sucrose molecules. The complex lacks pro-oxidant properties, thereby reducing oxidation of low- and very-low-density lipoproteins.

PHARMACOKINETICS

After intravenous administration, the drug enters the bloodstream via the lymphatic system. Maximum plasma concentration is reached after 24 hours. In the reticuloendothelial system, the complex is split into iron hydroxide and sucrose. In blood, iron binds to transferrin; in tissues, it is stored as ferritin; in bone marrow, it is incorporated into hemoglobin and used in erythropoiesis. Less than 1% of the dose is excreted by the kidneys.

CONTRAINDICATIONS

    • hypersensitivity; 
    • anemias not associated with iron deficiency (hemolytic, megaloblastic, due to vitamin B12 deficiency); 
    • disorders of erythropoiesis, bone marrow hypoplasia; 
    • iron overload in the body (hemochromatosis, hemosiderosis); 
    • impaired iron utilization (sideroachrestic anemia, thalassemia, lead anemia, late cutaneous porphyria); 
    • Osler–Rendu–Weber syndrome; 
    • chronic polyarthritis; 
    • bronchial asthma; 
    • acute infectious kidney diseases; 
    • uncontrolled hyperparathyroidism; 
    • decompensated liver cirrhosis; 
    • child before 18 years; 
    • pregnancy (first trimester); 
    • infectious hepatitis (due to accumulation of the drug in the reticuloendothelial system). 

With caution: hepatic and/or renal insufficiency, cardiovascular and allergic diseases.

ADVERSE REACTIONS

In rare cases: arthralgia, lymphadenopathy, fever, headache, general malaise, dyspepsia (nausea, vomiting), metallic taste in the mouth; very rarely — allergic reactions.

Local reactions (in case of improper administration technique): skin discoloration, pain, inflammation.

DRUG INTERACTIONS

Angiotensin-converting enzyme inhibitors enhance the systemic effects of the drug. Concomitant use with oral iron-containing preparations is not recommended, as iron absorption from the gastrointestinal tract is reduced. Treatment with oral iron preparations should be initiated no earlier than one week after the last injection.

SPECIAL PRECAUTIONS

The drug must not be administered intravenously.

Intramuscular administration may cause anaphylactic reactions due to hypersensitivity. Therefore, parenteral iron therapy should be used only in patients for whom oral therapy is insufficient and/or impossible. A sensitivity test is recommended before administration (test dose: 0.5 ml intramuscularly). In case of allergic reactions, epinephrine and glucocorticoids may be used. Use with caution in patients predisposed to allergic reactions.

Only intact ampoules should be used. The solution must not be used if a precipitate is present. After opening the ampoule, the solution should be administered immediately.

In anemia associated with malignant tumors or infectious diseases, iron is deposited in the reticuloendothelial system and can be utilized only after treatment of the underlying disease.

Effects on ability to drive and operate machinery: No effect.

Use during pregnancy and lactation:

Contraindicated in the first trimester of pregnancy. In the second and third trimesters, use is possible only if the expected benefit outweighs the potential risk to the fetus.

 Small amounts of unchanged iron may pass into breast milk; however, adverse effects in breastfed infants are unlikely.
