COMPOSITION

Each tablet contains: 

spiramycin 3,000,000 IU. 

INDICATIONS FOR USE 

Infectious and inflammatory diseases caused by microorganisms sensitive to the drug:

    • acute and chronic pharyngitis caused by group A β-hemolytic streptococcus (as an alternative to β-lactam antibiotics, particularly when their use is contraindicated); 
    • acute sinusitis (taking into account the susceptibility of the most common pathogens; use is indicated when β-lactam antibiotics are contraindicated); 
    • acute and chronic tonsillitis; 
    • acute bronchitis and exacerbation of chronic bronchitis; 
    • community-acquired pneumonia in patients without risk factors for poor outcome, severe clinical symptoms, or signs suggestive of pneumococcal etiology; 
    • pneumonia caused by atypical pathogens (Chlamydia pneumoniae, Chlamydia trachomatis, Mycoplasma pneumoniae, Legionella spp.) or suspected cases, regardless of severity or risk factors; 
    • skin and soft tissue infections, including impetigo, impetiginization, ecthyma, infectious dermohypodermitis (including erysipelas), secondary infected dermatoses, and erythrasma; 
    • oral cavity infections (including stomatitis, glossitis); 
    • non-gonococcal genital infections; 
    • toxoplasmosis, including during pregnancy; 
    • infections of the musculoskeletal system and connective tissue, including periodontitis. 

Prevention of recurrence of rheumatism in patients allergic to β-lactam antibiotics.

Eradication of Neisseria meningitidis from the nasopharynx (when rifampicin is contraindicated) for prophylaxis (not treatment) of meningococcal meningitis:

    • in patients after treatment and before release from quarantine; 
    • in patients who had contact within 10 days prior to hospitalization with individuals carrying Neisseria meningitidis. 

DOSAGE AND ADMINISTRATION

Dorymicin is administered orally with a sufficient amount of water.

Adults:  2–3 tablets of 3,000,000 IU (i.e., 6,000,000–9,000,000 IU) per day. The daily dose is divided into 2–3 administrations. The maximum daily dose is 9,000,000 IU.

Prevention of meningococcal meningitis:

    • Adults: 3,000,000 IU twice daily for 5 days. 

Treatment of toxoplasmosis in pregnancy:

Oral spiramycin 6,000,000–9,000,000 IU daily in 3-week courses with 2-week intervals significantly reduces the risk of fetal infection.

Renal impairment:

No dose adjustment is required due to minimal renal excretion.

DOSAGE FORM

Film-coated tablets. № 10. 

2 blisters together with a leaflet are packed in a carton box.

PHARMACOLOGICAL PROPERTIES

Doramycin is a macrolide antibiotic. It exerts a bacteriostatic effect by inhibiting intracellular protein synthesis. It penetrates and accumulates in phagocytic cells (neutrophils, monocytes, peritoneal and alveolar macrophages). Intracellular concentrations are sufficiently high, which explains its activity against intracellular pathogens.

Spiramycin is active against microorganisms with a minimum inhibitory concentration (MIC ≤ 1 mg/L), including more than 90% of susceptible strains: streptococci, methicillin-susceptible staphylococci, enterococci, Rhodococcus equi, Branhamella catarrhalis, Bordetella pertussis, Helicobacter pylori, Campylobacter jejuni, Legionella spp., Corynebacterium diphtheriae, Moraxella spp., Mycoplasma pneumoniae, Coxiella spp., Chlamydia spp., Treponema pallidum, Borrelia burgdorferi, Leptospira spp., Propionibacterium acnes, Actinomyces spp., Eubacterium spp., Porphyromonas spp., Mobiluncus spp., Bacteroides spp., Peptostreptococcus spp., Prevotella spp.

The antibiotic shows moderate in vitro activity; effects may occur at concentrations higher than MIC at the site of infection: Neisseria gonorrhoeae, Ureaplasma urealyticum, Clostridium perfringens.

Resistant microorganisms (MIC > 4 mg/L), at least 50% of strains: methicillin-resistant staphylococci, Enterobacter spp., Pseudomonas spp., Acinetobacter spp., Nocardia spp., Fusobacterium spp., Haemophilus spp., Mycoplasma hominis.

Spiramycin is active in vitro and in vivo against Toxoplasma gondii.

CONTRAINDICATIONS

    • hypersensitivity to the components of the medicinal product; 
    • glucose-6-phosphate dehydrogenase (G6PD) deficiency (risk of acute hemolysis); 
    • lactation period; 
    • pediatric use under 18 years. 

Precautions:

 Doramycin should be used with caution in patients with biliary obstruction and hepatic insufficiency.

ADVERSE REACTIONS

Gastrointestinal system: nausea, vomiting, diarrhea; very rarely (<0.01%) pseudomembranous colitis, abnormal liver function tests, cholestatic hepatitis; isolated cases of ulcerative esophagitis and acute colitis. Acute intestinal mucosal damage has also been reported in AIDS patients receiving high-dose spiramycin for cryptosporidiosis (2 cases reported).

Nervous system: in isolated cases, transient paresthesia.

Exanthematous pustulosis has also been reported.

DRUG INTERACTIONS

Concomitant use with medicinal products containing a combination of levodopa and carbidopa has been associated with a decrease in plasma levodopa levels.

SPECIAL PRECAUTIONS

During treatment, liver function should be periodically monitored in patients with hepatic disease.

If generalized erythema and pustular eruptions accompanied by high fever occur at the beginning of treatment, acute generalized exanthematous pustulosis (AGEP) should be suspected. In such cases, treatment must be discontinued, and further use of spiramycin, either as monotherapy or in combination, is contraindicated.

Use during pregnancy and lactation:

Doramycinn may be used during pregnancy when clinically indicated.

During lactation, breastfeeding should be discontinued, as spiramycin may be excreted in breast milk.
