COMPOSITION

 1 tablet contains:

  Domperidone — 10 mg. 

INDICATIONS FOR USE 

·Complex of dyspeptic symptoms frequently associated with delayed gastric emptying, gastroesophageal reflux, and esophagitis (feeling of epigastric fullness, abdominal bloating, upper abdominal pain, belching, nausea, vomiting, and heartburn). 

·Nausea and vomiting of functional, organic, or infectious origin caused by radiotherapy, drug therapy, or dietary disturbances. A specific indication is nausea and vomiting induced by dopamine agonists used in Parkinson’s disease (such as L-dopa and bromocriptine). 

·  In pediatric practice: regurgitation syndrome, cyclic vomiting, gastroesophageal reflux, and other disorders of gastric motility

DOSAGE AND ADMINISTRATION

Chronic dyspepsia:

    • Adults: 10 mg three times daily and, if necessary, at bedtime. 
    • Children: 2.5 mg per 10 kg body weight three times daily and, if necessary, at bedtime. 

Acute and subacute conditions:

    • Adults: 20 mg three to four times daily and at bedtime. 
    • Children: 5 mg per 10 kg body weight three to four times daily and at bedtime. 

Doprokin should be taken 15–30 minutes before meals.

PHARMACOLOGICAL PROPERTIES

Doprokin is an antiemetic medicinal product and a stimulant of gastrointestinal motility intended for oral administration. Doprokin is a dopamine antagonist with antiemetic properties.

The medicinal product does not cross the blood-brain barrier. Its antiemetic effect is due to a combination of peripheral (gastrokinetic) activity and antagonism of dopamine receptors in the chemoreceptor trigger zone.

Doprokin increases the duration of antral and duodenal contractions, accelerates gastric emptying, and increases the pressure of the lower esophageal sphincter without affecting gastric secretion.

Following oral administration on an empty stomach, Doprokin is rapidly absorbed. Maximum plasma concentration (Cmax) is reached within approximately 1 hour. Bioavailability is 15%. Plasma protein binding is 91–93%.

Doprokin is metabolized in the liver by hydroxylation and N-dealkylation. Urinary and fecal excretion account for 31% and 66%, respectively. The elimination half-life (T1/2) is 7–9 hours.

CONTRAINDICATIONS

Hypersensitivity to domperidone or any component of the medicinal product; gastrointestinal bleeding; mechanical obstruction or perforation of the gastrointestinal tract; prolactin-secreting pituitary tumor (prolactinoma).

ADVERSE REACTIONS

In isolated cases, transient intestinal spasms may occur; rarely, rash and urticaria have been reported.

DRUG INTERACTIONS

Anticholinergic agents may neutralize the antidyspeptic effect of Doprokin. Antacids and antisecretory agents reduce the bioavailability of Doprokin.

SPECIAL PRECAUTIONS

Special warnings and precautions:

Doprokin should be administered with caution in patients with hepatic insufficiency.

Use during pregnancy:

In animal studies, domperidone administered at doses up to 160 mg/kg/day did not demonstrate teratogenic effects. Doprokin should be used during the first trimester of pregnancy only when the expected therapeutic benefit justifies the potential risk to the fetus.
