About the Product

COMPOSITION

Each vial contains:

hydroxocobalamin — 10 mg. 

The ampoule with solvent contains:

diclofenac potassium 75 mg, betamethasone sodium phosphate 2.63 mg;

INDICATIONS FOR USE 

·exacerbation of rheumatism; 

·  inflammatory mono- and polyneuropathies

DOSAGE AND ADMINISTRATION

The medicinal product is administered intramuscularly in adults.

The contents of the vial containing the lyophilized powder should be dissolved using the contents of the ampoule with the solvent.

The injection should be administered deep intramuscularly at intervals of several hours 1 or 2 times daily (one injection into each gluteal muscle).

Clodizone should not be used for more than 2 days.

The medicinal product should be used at the maximum effective dose for the shortest possible duration, according to the individual treatment needs of the patient.

PHARMACOLOGICAL PROPERTIES

Clodizone is a combination medicinal product with anti-inflammatory, analgesic, anti-oedematous, and neurotrophic properties.

Diclofenac potassium (NSAID)

    • provides anti-inflammatory, analgesic, and antipyretic effects; 
    • inhibits cyclooxygenase enzymes COX-1 and COX-2; 
    • reduces the production of pain and inflammatory mediators. 

Betamethasone sodium phosphate (rapid-acting glucocorticosteroid)

    • provides potent anti-inflammatory and anti-oedematous effects; 
    • inhibits phospholipase A₂ activity; 
    • reduces vascular permeability and suppresses immune activity at the site of inflammation. 

Hydroxocobalamin (Vitamin B₁₂)

    • is converted into its active coenzyme form more rapidly than cyanocobalamin; 
    • remains in the bloodstream longer and is eliminated more slowly in the urine; 
    • improves metabolism and tissue regeneration; 
    • participates in DNA synthesis, myelin formation, and haematopoiesis; 
    • exhibits antineuritic activity.

CONTRAINDICATIONS

·hypersensitivity to the components of the medicinal product or other NSAIDs; 

·active gastric and duodenal ulcer disease; 

·gastrointestinal bleeding or perforation; 

·severe hepatic and renal impairment; 

·decompensated heart failure; 

·severe hypertension; 

·systemic mycosis; 

·active tuberculosis; 

·osteoporosis; 

·Cushing’s syndrome; 

·diabetes mellitus; 

·hepatitis A, hepatitis B, non-A/non-B hepatitis and other viral infections; 

·vaccination period; 

·pregnancy and breastfeeding period; 

·intramuscular administration in patients with idiopathic thrombocytopenic purpura; 

·gout; 

·treatment with systemic anticoagulants; 

·  pediatric age.

ADVERSE REACTIONS

The following adverse reactions may occur:

    • allergic reactions (rash, pruritus, urticaria, angioedema, anaphylactic reactions); 
    • pain, redness, or induration at the injection site; 
    • nausea, vomiting, abdominal pain, dyspepsia; 
    • gastrointestinal ulceration and bleeding; 
    • headache, dizziness; 
    • increased blood pressure; 
    • fluid retention and peripheral oedema; 
    • elevated liver enzyme levels; 
    • renal impairment; 
    • haematological disorders (leukopenia, thrombocytopenia, anaemia); 
    • with prolonged exposure to the glucocorticosteroid component: hyperglycaemia, Cushing's syndrome, osteoporosis, immunosuppression, and an increased risk of infections.

DRUG INTERACTIONS

·Other NSAIDs and corticosteroids may increase the risk of gastrointestinal ulceration and bleeding. 

·Anticoagulants and antiplatelet agents increase the risk of bleeding. 

·Antihypertensive agents and diuretics may have reduced efficacy, with an increased risk of renal impairment. 

·Methotrexate may have increased toxicity. 

·Lithium preparations may show increased plasma lithium concentrations. 

·Cyclosporine and tacrolimus may increase the risk of nephrotoxicity. 

·Antidiabetic medicines may become less effective due to the glucocorticosteroid component, resulting in elevated blood glucose levels. 

·  Live vaccines should not be administered concomitantly because of the increased risk of infection.

SPECIAL PRECAUTIONS

    • The medicinal product should be used at the lowest effective dose for the shortest possible duration
    • Patients with gastrointestinal, hepatic, renal, or cardiovascular disorders should be carefully monitored during treatment. 
    • Treatment should be discontinued immediately if signs of gastrointestinal bleeding occur. 
    • During prolonged treatment, blood pressure, liver and kidney function, and blood counts should be monitored. 
    • Caution is advised when driving or operating machinery if dizziness or drowsiness occurs. 
    • Alcohol consumption should be avoided during treatment.
