COMPOSITION

Magnesium hydroxide 15.2mg. 

INDICATIONS FOR USE 

  • Primary prevention of cardiovascular diseases such as thrombosis and acute coronary syndrome in the presence of risk factors: arterial hypertension, hypercholesterolemia, diabetes mellitus, obesity (body mass index > 30), smoking, advanced age, and a family history of myocardial infarction before the age of 55;
  • Prevention of recurrent myocardial infarction and vascular thrombosis;
  • Prevention of thromboembolism after vascular surgical interventions (coronary artery bypass grafting, percutaneous transluminal coronary angioplasty);
  • Unstable angina. 

DOSAGE AND ADMINISTRATION

Tablets are taken orally, swallowed whole with water. If necessary, they may be divided, chewed, or dispersed in water for faster absorption.

The duration of treatment is determined individually.

    • Primary prevention of cardiovascular diseases: 150 mg on the first day, then 75 mg once daily. 
    • Prevention of recurrent myocardial infarction and thrombosis: 75–150 mg/day. 
    • Prevention of thromboembolism after vascular surgery: 75–150 mg/day. 
    • Unstable angina: 75–150 mg/day.

PHARMACOLOGICAL PROPERTIES

Acetylsalicylic acid (ASA) has analgesic, anti-inflammatory, antipyretic and antiplatelet effects. Its main mechanism is irreversible inhibition of COX-1, leading to suppression of thromboxane A₂ synthesis and platelet aggregation. The effect persists throughout the platelet lifespan.

Magnesium hydroxide acts as an antacid and protects the gastrointestinal mucosa from the irritant effect of ASA.

After oral administration, ASA is rapidly absorbed with a bioavailability of about 70%. Peak plasma concentrations are reached within 10–20 minutes (ASA) and 0.3–2 hours (salicylic acid). Plasma protein binding is 66–90%.

ASA is metabolized in the liver to salicylic acid and excreted via the kidneys. The half-life is dose-dependent: 2–3 hours at low doses and 15–30 hours at high doses.

Magnesium is minimally absorbed, partly excreted in urine and mainly eliminated in feces.

CONTRAINDICATIONS

  • Hypersensitivity to acetylsalicylic acid (ASA), other components of the drug, or NSAIDs;
  • Cerebral hemorrhage;
  • Tendency to bleeding (vitamin K deficiency, thrombocytopenia, hemophilia);
  • Bronchial asthma or angioedema induced by salicylates or NSAIDs in the medical history;
  • Erosive and ulcerative gastrointestinal lesions (in the acute phase);
  • Gastrointestinal bleeding;
  • Severe renal impairment (creatinine clearance < 10 ml/min);
  • Severe hepatic impairment (Child-Pugh class B and above);
  • Severe heart failure;
  • First and third trimesters of pregnancy;
  • Glucose-6-phosphate dehydrogenase deficiency;
  • Concomitant use with methotrexate (> 15 mg/week);
  • Children and adolescents under 18 years of age.

ADVERSE REACTIONS

Asmada (acetylsalicylic acid + magnesium hydroxide) is an antiplatelet agent whose adverse effects are most commonly associated with irritation of the gastrointestinal tract.

The main risks include gastrointestinal bleeding, ulcer formation, bronchospasm, dizziness, tinnitus, impaired liver and kidney function, and allergic reactions (skin rash, angioedema).

Main adverse effects:

    • Gastrointestinal tract: nausea, heartburn, epigastric pain, diarrhea; possible erosive and ulcerative lesions of the stomach and intestines; increased risk of bleeding, including occult bleeding. 
    • Respiratory system: may provoke bronchospasm and asthma attacks. 
    • Nervous system: headache, insomnia, dizziness, tinnitus, somnolence.

DRUG INTERACTIONS

With concomitant use, Asmada enhances the effect of hypoglycemic agents, heparin and indirect anticoagulants, thrombolytic and antiplatelet agents, methotrexate, digoxin, and valproic acid.

ASA reduces the effect of diuretics, angiotensin-converting enzyme (ACE) inhibitors, and uricosuric agents (benzbromarone, probenecid). By increasing the elimination of salicylates, systemic glucocorticosteroids weaken their effect.

Concomitant use of Asmada and ibuprofen reduces the cardioprotective effect of ASA.

Antacids and cholestyramine reduce the absorption of the drug. The clinical significance of the interaction between ASA and magnesium hydroxide included in the formulation is minimal due to the low content of magnesium hydroxide.

SPECIAL PRECAUTIONS

Prolonged concomitant use of Asmada with NSAIDs should be avoided due to an increased risk of adverse effects. ASA may provoke bronchospasm and trigger asthma attacks. It may also cause bleeding of varying severity during and after surgical procedures; therefore, in case of planned surgery, it is recommended to discontinue the drug several days in advance.

Low doses of ASA may precipitate gout in predisposed individuals. High doses have a hypoglycemic effect, which should be considered in patients with diabetes receiving hypoglycemic therapy. Overdose of ASA is associated with an increased risk of gastrointestinal bleeding.

Concomitant use of ASA with methotrexate increases the risk of hematological adverse effects. When used together with systemic glucocorticosteroids, salicylate levels in the blood decrease during treatment, while after discontinuation of corticosteroids, salicylate overdose may occur.

The combination of Asmada with ibuprofen is not recommended, as it may reduce the antiplatelet effect of ASA (at doses up to 300 mg), thereby decreasing its cardioprotective effects.

Concomitant use with alcohol increases the risk of gastrointestinal mucosal damage and prolongation of bleeding time.
