About the Product

COMPOSITION

1 ml of the product contains:

Active ingredients:

    • xylometazoline hydrochloride — 1.0 mg; 
    • dexpanthenol — 50 mg. 

INDICATIONS FOR USE 

·relief of nasal congestion associated with common cold conditions; 

·supportive therapy for healing of damaged nasal mucosa occurring with rhinitis (vasomotor rhinitis); 

·restoration of nasal breathing after surgical procedures in the nasal cavity; 

·  reduction of nasal mucosal swelling in acute respiratory diseases accompanied by inflammation of the paranasal sinuses (rhinosinusitis)

DOSAGE AND ADMINISTRATION

Adults and children over 6 years of age should use one spray into each nostril up to 3 times daily.

The dosage depends on individual sensitivity and clinical response.

Method of administration

    • For intranasal use. 
    • Keep the bottle in a vertical position during spraying. 

Frequency of use

    • One spray into each nostril no more than 3 times a day. 

Duration of treatment

    • The product should not be used for more than 7 days unless recommended by a physician. 
    • A doctor should be consulted regarding use in children. 
    • Repeated treatment should only be started after a break of several days.

PHARMACOLOGICAL PROPERTIES

Pharmacokinetics

Xylometazoline

 When applied locally, it is practically not absorbed. Plasma concentrations are so low that they cannot be detected by modern analytical methods.

Dexpanthenol

 After topical application, it is rapidly absorbed and converted into pantothenic acid, which binds to plasma proteins, mainly β-globulin and albumin. Its concentration is approximately 0.5–1 mg/L in blood and about 100 µg/L in serum. Pantothenic acid is not metabolized in the body (except for incorporation into coenzyme A) and is excreted unchanged.

Pharmacodynamics

Xylometazoline

 Belongs to the group of topical vasoconstrictors (decongestants) with α-adrenergic activity. It constricts blood vessels of the nasal mucosa, reduces swelling and hyperemia, restores nasal patency, and facilitates nasal breathing. The effect usually begins within a few minutes after application and lasts up to 10 hours.

Dexpanthenol

 A derivative of pantothenic acid (vitamin B group). It stimulates regeneration of mucous membranes, normalizes cellular metabolism, accelerates mitosis, and increases the strength of collagen fibers. It has regenerative, metabolic, and mild anti-inflammatory effects.

CONTRAINDICATIONS

    • hypersensitivity to the active substances or any of the excipients; 
    • dry inflammation of the nasal mucosa (rhinitis sicca); 
    • increased intraocular pressure, especially angle-closure glaucoma (only for Xylazol nasal spray 1 mg/50 mg); 
    • history of transsphenoidal hypophysectomy or other surgical procedures involving exposure of the meninges; 
    • children under 6 years of age. 

Due to the presence of benzalkonium chloride as a preservative, the product should not be used in patients with known hypersensitivity to this substance.

ADVERSE REACTIONS

Uncommon:

    • epistaxis (nosebleed); 
    • hypersensitivity reactions (angioedema, rash, pruritus). 

Rare:

    • palpitations; 
    • tachycardia; 
    • hypertension. 

Very rare:

    • arrhythmias; 
    • increased swelling of the nasal mucosa after the effect of the medicine wears off; 
    • fatigue (drowsiness, sedation); 
    • headache; 
    • seizures (especially in children); 
    • restlessness; 
    • insomnia; 
    • hallucinations (mainly in children). 

DRUG INTERACTIONS

Xylometazoline hydrochloride

    • Concomitant use with monoamine oxidase inhibitors (MAOIs), including tranylcypromine, tricyclic antidepressants, or medicines that increase blood pressure may lead to elevated blood pressure due to their effects on the cardiovascular system. 

Dexpanthenol

    • No data are available regarding interactions between dexpanthenol and other medicinal products.

SPECIAL PRECAUTIONS

In chronic rhinitis, the product should only be used under medical supervision because of the risk of nasal mucosal atrophy.

When using sympathomimetic agents that reduce mucosal swelling, especially with prolonged use or overdose, reactive hyperemia of the nasal mucosa may occur.

This rebound effect causes narrowing of the nasal passages, leading the patient to use the product repeatedly and eventually continuously.

As a result, the following conditions may develop:

    • chronic swelling of the nasal mucosa (rhinitis medicamentosa); 
    • atrophy of the nasal mucosa; 
    • atrophic fetid rhinitis (ozena). 

In mild cases, it may be sufficient to discontinue the sympathomimetic first in one nostril and, after symptoms improve, in the other nostril, while maintaining at least partial nasal breathing.
