About the Product
COMPOSITION
Each 5 mL of suspension contains:
- Aluminum hydroxide — 405 mg;
- Magnesium hydroxide — 100 mg;
- Simethicone — 125 mg.
INDICATIONS FOR USE
·symptomatic treatment of dyspepsia;
·symptomatic treatment of heartburn;
· symptomatic treatment of flatulence.
DOSAGE AND ADMINISTRATION
Adults and children over 5 years of age:
- 5–10 mL 20–60 minutes after meals and at bedtime, or as needed.
Children under 5 years of age:
- no more than 5 mL three times daily.
Elderly patients:
- the usual adult dose is recommended.
DOSAGE FORM
Oral suspension, 250 mL in a bottle.
PHARMACOLOGICAL PROPERTIES
Pharmacodynamics
Simalgel is a combination of two antacids—aluminum hydroxide and magnesium hydroxide—and the antiflatulent (antifoaming) agent simethicone.
Magnesium hydroxide is a fast-acting antacid, whereas aluminum hydroxide is a slow-acting antacid. Their combination provides a rapid onset and prolonged duration of action.
Aluminum hydroxide has an astringent effect and may slow intestinal motility; however, this effect is offset by magnesium hydroxide, which has a mild laxative effect.
Simethicone prevents the formation of gas bubbles and promotes their breakdown. The released gases are absorbed through the intestinal wall and eliminated by intestinal peristalsis.
Pharmacokinetics
The drug is practically not absorbed and does not disturb the body's electrolyte balance.
CONTRAINDICATIONS
·hypersensitivity to any component of the drug;
·impaired renal function;
· children under 10 years of age.
ADVERSE REACTIONS
- pruritus;
- urticaria;
- angioedema;
- anaphylactic reactions;
- diarrhea or constipation;
- hypermagnesemia;
- hyperaluminemia;
- hypophosphatemia.
DRUG INTERACTIONS
·An interval of at least 1 hour should be maintained between Simalgel and other medications, as simultaneous administration may reduce the absorption of other drugs.
· Concomitant use of aluminum hydroxide and citrate-containing products may increase aluminum levels in the body, particularly in patients with renal impairment.
SPECIAL PRECAUTIONS
Aluminum hydroxide may cause constipation, while an overdose of magnesium salts may lead to decreased intestinal motility (intestinal hypokinesia). High doses of the product may trigger or worsen intestinal obstruction in high-risk patients (patients with renal impairment and elderly patients).
Aluminum hydroxide is poorly absorbed from the gastrointestinal tract; therefore, systemic effects are rarely observed in patients with normal renal function.
