INDICATIONS FOR USE 

Muscomed is indicated for the relief of spasm-related pain in:

• • degenerative diseases of the spine (dorsalgia, lumbalgia, etc.); 
  • traumatic and neurological conditions accompanied by muscle spasms. 

DOSAGE AND ADMINISTRATION

Muscomed is taken orally after meals. Consultation with a physician is recommended before use.

The maximum daily dose is 8–16 mg, divided into 2 doses. Treatment duration is 5–7 days.

When preparing for physiotherapy procedures, the period of muscle relaxation is 1–2 hours after taking the drug.

PHARMACOLOGICAL PROPERTIES

Muscomed contains thiocolchicoside as the active substance, a semi-synthetic muscle relaxant derived from the natural glycoside colchicoside. Thiocolchicoside shows selective affinity for gamma-aminobutyric acid (GABA) and glycine receptors, which explains its action at different levels of the nervous system in both peripheral and central muscle spasms.

Muscomed has no curare-like effect, does not cause paralysis, and does not affect the cardiovascular or respiratory systems.

Pharmacokinetics

After oral administration, thiocolchicoside is first metabolized into the aglycone 3-demethylthiocolchicine (SL59.0955). This process occurs via intestinal metabolism, explaining the absence of unchanged thiocolchicoside in plasma. SL59.0955 is then glucuronidated to form the pharmacologically active metabolite SL18.0740.

Maximum plasma concentrations of SL59.0955 and SL18.0740 are reached 1 hour after administration. After a single 8 mg oral dose, Cmax and AUC of SL18.0740 are approximately 60 ng/mL and 130 ng·h/mL, respectively. For SL59.0955, values are lower: Cmax ~13 ng/mL, AUC ranges from 15.5 ng·h/mL (up to 3 h) to 39.7 ng·h/mL (up to 24 h).

The elimination half-life is 3.2–7 hours for SL18.0740 and 0.8 hours for SL59.0955.

After oral administration, about 20% of the dose is excreted via urine and 75–81% via feces.

CONTRAINDICATIONS

·hypersensitivity to any component of the drug; 

·muscle hypotonia; 

·mild paralysis; 

·pregnancy; 

·lactation period; 

·  children under 16 years of age.

ADVERSE REACTIONS

Rare: skin rash, edema, allergic and pseudo-allergic reactions (allergic vasculitis), gastralgia, diarrhea.

Very rare: hypotension, irritability, and drowsiness.

DRUG INTERACTIONS

There are no data on drug interactions. However, caution is recommended when used concomitantly with other muscle relaxants.

SPECIAL PRECAUTIONS

Muscomed should be used with caution in patients with impaired renal or hepatic function. Caution is also required in patients with epilepsy or a risk of seizures.
