COMPOSITION

1 tablet contains: 

meloxicam 15 mg. 

INDICATIONS FOR USE 

Indicated for the symptomatic treatment of pain in the following conditions:

- rheumatoid arthritis, ankylosing spondylitis (Bechterew's disease), deforming osteoarthrosis of the joints, sprains, and bruises;

- radiculitis, neuritis, and neuralgia;

- dysmenorrhea, associated with antibiotic therapy for pelvic inflammatory diseases.

 

DOSAGE AND ADMINISTRATION

The total daily dose is taken once daily, with water or other liquid, during meals.

Exacerbation of osteoarthritis: 7.5 mg meloxicam per day. If necessary, the dose may be increased to 15 mg per day.

Rheumatoid arthritis and ankylosing spondyloarthritis: 15 mg meloxicam per day. Depending on clinical response, the dose may be reduced to 7.5 mg per day.

For long-term treatment in elderly patients, a dose of 7.5 mg per day is recommended.

Treatment of patients at increased risk of adverse reactions should be initiated at 7.5 mg per day.

In patients with renal impairment undergoing haemodialysis, the daily dose should not exceed 7.5 mg meloxicam.

PHARMACOLOGICAL PROPERTIES

Meloxicam is an enolic acid derivative belonging to the class of non-steroidal anti-inflammatory drugs (NSAIDs). It exerts pronounced anti-inflammatory, analgesic, and antipyretic effects. High anti-inflammatory activity has been demonstrated in all standard models of inflammation. The mechanism of action is related to inhibition of prostaglandin biosynthesis (inflammatory mediators) through selective inhibition of COX-2, providing a more favourable safety profile. The therapeutic effect of NSAIDs is associated with COX-2 inhibition, whereas COX-1 inhibition is responsible for gastrointestinal and renal adverse effects.

The COX-2 selectivity of meloxicam has been confirmed both in vitro and in vivo. Meloxicam does not affect platelet aggregation or bleeding time at recommended doses ex vivo, unlike indomethacin, diclofenac, ibuprofen, and naproxen, which significantly inhibit platelet aggregation and prolong bleeding time.

Pharmacokinetics

Meloxicam is well absorbed from the gastrointestinal tract (GIT), as evidenced by high absolute bioavailability (89%).

After a single oral dose, the mean peak plasma concentration is reached within 5–6 hours.

Meloxicam is highly bound to plasma proteins, mainly albumin (99%). It penetrates into synovial fluid, where concentrations are approximately 50% of plasma levels. The volume of distribution is low, averaging about 11 litres.

Meloxicam is almost completely metabolized in the liver into four pharmacologically inactive metabolites. The main metabolite, 5'-carboxymeloxicam (60% of the dose), is formed via oxidation of the intermediate metabolite 5'-hydroxymethylmeloxicam (9% of the dose).

Meloxicam is eliminated equally via faeces and urine, mainly as metabolites. Less than 5% of the dose is excreted unchanged in faeces, and only trace amounts are found in urine.

The mean elimination half-life is about 20 hours. Plasma clearance is approximately 8 mL/min.

In elderly patients, plasma clearance is slightly reduced.

CONTRAINDICATIONS

·known hypersensitivity to meloxicam or any component of the product. There is a possibility of cross-sensitivity to acetylsalicylic acid and other NSAIDs. The product must not be administered to patients who previously experienced bronchial asthma, nasal polyps, angioedema, or urticaria after taking acetylsalicylic acid or other NSAIDs; 

·history of gastrointestinal bleeding or perforation related to NSAID use; 

·active gastric or duodenal ulcer; 

·severe hepatic impairment; 

·severe renal impairment (unless on haemodialysis); 

·active gastrointestinal bleeding, recent cerebrovascular bleeding, or other active bleeding; 

·severe heart failure; 

·third trimester of pregnancy and lactation period; 

·  children under 15 years of age.

ADVERSE REACTIONS

Dyspepsia, nausea, vomiting, abdominal pain, constipation, flatulence, diarrhoea, allergic reactions.

DRUG INTERACTIONS

Concomitant use with acetylsalicylic acid administered at anti-inflammatory doses is not recommended. Combined use with anticoagulants and heparin increases the risk of bleeding due to inhibition of platelet function and damage to the gastric and intestinal mucosa.

SPECIAL PRECAUTIONS

Adverse reactions may be minimized by using the lowest effective dose for the shortest duration necessary to control symptoms. Meloxicam is not recommended for patients with severe pain.

If no improvement in symptoms is observed after several days of treatment, the clinical effectiveness of the therapy should be reassessed.
