COMPOSITION

Each 5 ml of syrup contains:

Inosine pranobex — 250 mg. 

INDICATIONS FOR USE 

·viral infectious diseases in patients with normal or weakened immune status, including influenza, parainfluenza, acute respiratory viral infections (ARVI), viral bronchitis, rhinovirus and adenovirus infections, mumps, and severe or complicated measles; 

·diseases caused by Herpes simplex virus type I and type II, including herpes labialis, facial herpes, oral mucosal herpes, herpes of the hands, ophthalmic herpes, subacute sclerosing panencephalitis, and genital herpes; 

·diseases caused by Varicella zoster virus, including chickenpox and herpes zoster (shingles), including recurrent forms in immunocompromised patients; 

·infections caused by the Epstein–Barr virus (infectious mononucleosis); 

·cytomegalovirus infection; 

·infections caused by human papillomavirus (HPV); 

·acute and chronic hepatitis B virus infection; 

·  chronic recurrent respiratory and genitourinary tract infections in immunocompromised patients, including chlamydial infections and other diseases caused by intracellular pathogens

DOSAGE AND ADMINISTRATION

The product is administered orally, preferably after meals, at regular intervals.

The maximum daily dose for adults is 80 ml of syrup, equivalent to 4 g of inosine pranobex.

The usual daily dose for children is 50 mg/kg body weight, divided into three doses per day (according to the dosing table).

If necessary, particularly in severe cases, the recommended daily dose may be increased as directed by a physician.

PHARMACOLOGICAL PROPERTIES

Pharmacodynamics

The drug has direct antiviral and immunomodulatory effects.

The immunomodulatory action is associated with effects on T-lymphocytes (activation of cytokine synthesis) and increased phagocytic activity of macrophages. The drug enhances differentiation of pre-T lymphocytes, proliferation of T- and B-lymphocytes, and the ability of T-lymphocytes to synthesize lymphokines. The ratio between T-helper and T-suppressor subpopulations is normalized.

Inoseda significantly increases the production of interleukin-1 and interleukin-2 by lymphocytes and promotes receptor expression on lymphoid cells. It stimulates natural killer (NK) cell activity, enhances macrophage phagocytosis, antigen processing and presentation, leading to an increase in antibody-producing cells from the first days of treatment.

The drug also prevents post-viral suppression of RNA and protein synthesis in infected cells, which is particularly important for immune system cells.

As a result, viral load is reduced, immune system function is normalized, endogenous interferon production is enhanced, and resistance to infections is increased with faster localization of infectious foci.

Pharmacokinetics

Absorption: Inosine pranobex has high bioavailability. Peak plasma concentration of inosine is reached in about 1 hour, onset of action occurs within 30 minutes and lasts up to 6 hours.

No accumulation in the body has been observed. Complete elimination of the drug and its metabolites occurs within 48 hours.

CONTRAINDICATIONS

·hypersensitivity to the active substance or any of the excipients of the product; 

·acute gout attack; 

·  hyperuricemia.

ADVERSE REACTIONS

·headache; 

·nausea, vomiting; 

·epigastric pain; 

·allergic reactions; 

·weakness; 

·joint pain; 

·increased activity of aminotransferases, alkaline phosphatase, or blood nitrogen levels; 

·hyperuricemia; 

·drowsiness or insomnia; 

·diarrhea; 

·constipation; 

·  polyuria.

DRUG INTERACTIONS

Immunosuppressive agents may reduce the effect of Inoseda.

Caution is advised in patients receiving xanthine oxidase inhibitors, drugs that increase uric acid excretion, and diuretics.

The drug should not be used concomitantly with immunosuppressant’s due to possible pharmacokinetic interactions, which may reduce the expected therapeutic effect.

SPECIAL PRECAUTIONS

The drug is metabolized to uric acid and may cause a significant increase in its concentration in urine.

It should be used with caution. If necessary, patients may be prescribed medications that reduce uric acid levels.
