About the Product

COMPOSITION

5 g of granules contain:

calcium-magnesium phytate – 125 mg;

calcium gluconate – 375 mg;

vitamin D2 – 3000 IU. 

INDICATIONS FOR USE 

Prevention and treatment of rickets; disorders of calcium metabolism accompanied by osteopathy, spasmophilia, impaired bone formation, delayed tooth eruption, and growth retardation during periods of intensive body development.

Prevention of osteoporosis of various origins.

Conditions associated with increased requirements for vitamin D and calcium:

    • breastfed children and/or children with insufficient exposure to fresh air; 
    • formula-fed children with vitamin D deficiency; 
    • malabsorption syndrome, chronic diarrhea, gastrectomy; 
    • exhaustion of the nervous system; 
    • steatorrhea; 
    • patients receiving hormonal therapy, anticonvulsants, immunosuppressants, aluminum-containing antacids, or diuretics. 

DOSAGE AND ADMINISTRATION

D-Calcin is administered orally.

Prevention of osteoporosis: 1000–2000 IU per day (¼–½ measuring cup).

Prevention of rickets and osteomalacia: 400 IU per day (1 teaspoon).

Treatment of vitamin D deficiency in diet, rickets, and osteomalacia:

    • Adults: 1000–2000 IU per day (¼–½ measuring cup). 
    • Children: 1000–4000 IU per day (¼–1 measuring cup). 

Treatment should be continued for several months. After completion of the treatment course, if necessary, switch to prophylactic doses of vitamin D (400 IU) for both adults and children.

Treatment of osteomalacia in patients receiving long-term anticonvulsant therapy:

    • Adults: 1000–4000 IU per day (¼–1 measuring cup). 
    • Children: 1000 IU per day (¼ measuring cup). 

Since sensitivity to vitamin D may vary, dosage should be adjusted according to clinical response. According to general standards, serum Ca × P (mg/dL) should not exceed 58.

PHARMACOLOGICAL PROPERTIES

Pharmacodynamics:

 D-Calcin is a combination of minerals and vitamin D2 that regulates calcium-phosphorus metabolism.

Calcium-magnesium phytate (phytin) is a complex organic phosphorus compound containing a mixture of calcium and magnesium salts. It promotes the growth and development of bone tissue. Phosphorus is an essential element included in proteins, nucleic acids, and bone tissue. Phosphorus compounds participate in energy metabolism (adenosine triphosphate and creatine phosphate are energy accumulators); muscular and mental activity, as well as vital body functions, are associated with their transformation. Phosphorus also affects cardiac and renal function.

Calcium gluconate replenishes calcium deficiency in the body. Calcium forms the basis of bone tissue, activates the activity of several important enzymes, participates in maintaining ionic balance in the body, and affects processes occurring in the neuromuscular and cardiovascular systems.

Vitamin D2 regulates calcium and phosphorus metabolism in the body. Its active metabolites (particularly calcitriol) easily penetrate cell membranes and bind to specific receptors in target organ cells, promoting activation of calcium-binding protein synthesis, facilitating intestinal absorption of calcium and phosphorus, enhancing their reabsorption in the proximal renal tubules, increasing uptake by bone tissue, and preventing bone resorption.

Pharmacokinetics:

 The effect of the medicinal product is due to the combined properties of its components; therefore, pharmacokinetic studies are not feasible.

CONTRAINDICATIONS

Hypersensitivity to the components of the medicinal product, renal osteodystrophy with hyperphosphatemia, severe renal insufficiency, predisposition to thrombosis, severe hypercalcemia, hypervitaminosis D, hyperparathyroidism.

ADVERSE REACTIONS

The medicinal product is generally well tolerated.

The following adverse reactions may occur during treatment: nausea, vomiting, diarrhea, bradycardia, and allergic reactions. Hypercalcemia may develop during prolonged use.

DRUG INTERACTIONS

The toxic effects are reduced by vitamin A, tocopherol, ascorbic acid, pantothenic acid, thiamine, and riboflavin.

In hypervitaminosis D, the effects of cardiac glycosides may be enhanced, with an increased risk of arrhythmias due to the development of hypercalcemia; dose adjustment of cardiac glycosides may therefore be required.

Under the influence of barbiturates (including phenobarbital), phenytoin, and primidone, the requirement for ergocalciferol may significantly increase due to accelerated metabolism.

Long-term therapy combined with aluminum- and magnesium-containing antacids increases their blood concentrations and the risk of intoxication.

Calcitonin, derivatives of etidronic and pamidronic acids, plicamycin, gallium nitrate, and glucocorticoids reduce the effect of the medicinal product.

SPECIAL PRECAUTIONS

The medicinal product should be used with caution in patients with renal insufficiency.

It should be taken into account that sensitivity to vitamin D varies among patients, and in some individuals even therapeutic doses may cause manifestations of hypervitaminosis. Sensitivity of newborns to vitamin D may also vary; some infants may be sensitive even to very low doses.

In elderly patients, the requirement for vitamin D may increase due to reduced absorption of vitamin D, decreased exposure to sunlight, and increased incidence of renal insufficiency.

The medicinal product should be used with caution in patients with active tuberculosis or sarcoidosis because of the possible calcification of granulomas.

Use during pregnancy and lactation:

High doses of vitamin D should not be used during pregnancy because overdose may produce teratogenic effects.

D-Calcin should be prescribed with caution to breastfeeding women, since high doses taken by the mother may cause symptoms of overdose in the infant.

Use in pediatrics:

The medicinal product may be used in pediatric practice.
