PHARMACOLOGICAL PROPERTIES

Pharmacological action

Butamirate, the active substance of Butara, is a centrally acting antitussive agent that is neither chemically nor pharmacologically related to opium alkaloids. It does not cause dependence or habituation.

It suppresses cough by directly acting on the cough center. It also has a bronchodilatory effect. It helps ease breathing by improving spirometry parameters (reducing airway resistance) and blood oxygenation (increasing oxygen saturation).

CONTRAINDICATIONS

    • hypersensitivity to the components of the product;
    • children under 2 months of age (for drops);
    • first trimester of pregnancy;
    • breastfeeding period;
    • fructose intolerance (the product contains sorbitol).

Precautions:

 Second and third trimesters of pregnancy. Due to the presence of ethanol, the product should be used with caution in patients prone to drug dependence, with liver diseases, alcoholism, epilepsy, brain disorders, pregnant women (2nd–3rd trimesters), and children.

Use during pregnancy and lactation:

Controlled clinical studies in pregnant women have not been conducted. Therefore, the product should not be used during the first trimester. In the second and third trimesters, it should be used only if the benefit to the mother outweighs the potential risk to the fetus.

 Due to the lack of data on excretion of butamirate in breast milk, use during lactation is not recommended.

ADVERSE REACTIONS

Frequency classification of adverse reactions:

very common (≥1/10), common (≥1/100 and <1/10), uncommon (≥1/1000 and <1/100), rare (≥1/10,000 and <1/1000), very rare (<1/10,000), including isolated reports.

Adverse effects:

Nervous system:

    • rare — drowsiness

Gastrointestinal system:

    • rare — nausea, diarrhea

Allergic reactions:

    • rare — urticaria
    • other hypersensitivity reactions may occur.

DRUG INTERACTIONS

In most cases, concomitant use of butamirate with other medicinal products does not result in clinically significant interactions.

However:

    • concomitant use with expectorants or mucolytics may lead to mucus accumulation in the airways due to suppression of the cough reflex;
    • combined use with CNS depressants (e.g., sedatives, hypnotics, analgesics) may enhance sedative effects;
    • due to ethanol content (in some formulations), caution is advised when used with drugs affecting the liver and central nervous system.

SPECIAL PRECAUTIONS

Butamirate should be used only for dry, non-productive cough. If sputum appears, the use of antitussive agents may hinder its clearance.

If cough persists for more than 7 days or worsens, medical advice should be sought.

The product should not be used concomitantly with expectorants or mucolytics without medical supervision.

Use with caution in patients with liver disease, epilepsy, a tendency to drug dependence, and in formulations containing ethanol.
