COMPOSITION
Each dose contains: Beclometasone dipropionate — 0.1 mg.
INDICATIONS FOR USE
Before use, the bottle should be shaken and sprayed into the air until an aerosol is formed. Beclomil spray is intended for intranasal use only.
Adults and children over 6 years of age are prescribed 1 metered spray (100 mcg) into each nostril 1–2 times daily. The maximum daily dose is 400 mcg.
The lowest effective dose that ensures control of clinical symptoms should be used. After achieving a therapeutic effect, the drug should be discontinued gradually with dose reduction.
DOSAGE AND ADMINISTRATION
Prevention and treatment of perennial and seasonal allergic rhinitis, including rhinitis associated with hay fever and vasomotor rhinitis.
DOSAGE FORM
Metered nasal spray 100 mcg/dose. 200 doses of suspension in a yellow plastic bottle with a metering device.
PHARMACOLOGICAL PROPERTIES
Beclomil is a topical glucocorticosteroid with anti-inflammatory and antiallergic effects. It inhibits the release of inflammatory mediators and increases the production of lipocortin (annexin), an inhibitor of phospholipase A2, leading to a reduction in arachidonic acid formation and its metabolic products.
Under the action of beclomethasone, the number of mast cells in the nasal mucosa decreases, marginal neutrophil accumulation is reduced, inflammatory exudate and cytokine production are decreased. Macrophage migration is also inhibited, and the intensity of infiltration and granulation processes is reduced.
CONTRAINDICATIONS
- Hypersensitivity to any component of the drug
- Pulmonary tuberculosis
- Children under 4 years of age
- Beclomethasone containing 250 mcg per dose is not intended for pediatric use (i.e., under 18 years of age)
With caution:
Use in patients with glaucoma, systemic infections (bacterial, viral, fungal, parasitic), osteoporosis, liver cirrhosis, hypothyroidism, pregnancy, and lactation.
ADVERSE REACTIONS
The frequency categories of adverse reactions used below are defined as follows: common (>1/100 to <1/10), very rare (<1/10,000).
Respiratory system:
Common — epistaxis, dryness and irritation of the nasal and throat mucosa;
Very rare — nasal septum perforation.
Sensory organs:
Common — unpleasant taste in the mouth;
Very rare — glaucoma, increased intraocular pressure, cataract.
Allergic reactions:
Common — rash, urticaria, pruritus, erythema;
Very rare — angioedema, bronchospasm, anaphylactic reactions.
DRUG INTERACTIONS
“Beкlomil” (active substance — beclomethasone) is a topical glucocorticosteroid (GCS). When used via inhalation or intranasal administration, its systemic effects are minimal; therefore, it has a limited number of clinically significant drug interactions.
However, certain interaction features should be noted:
Potentiation of beta-agonists:
Beclomethasone restores bronchial sensitivity to beta-agonists (e.g., salbutamol, fenoterol), which may reduce the need for frequent use.
Risk of adrenal suppression:
Concomitant use with other systemic or intranasal corticosteroids may enhance suppression of adrenal cortex function.
Enzyme inducers/inhibitors interaction:
Drugs affecting hepatic enzymes (e.g., ritonavir, itraconazole) may theoretically alter beclomethasone metabolism, although this is rarely clinically significant with local use.
Reduced efficacy:
Phenobarbital, phenytoin, and rifampicin may reduce the therapeutic effect of beclomethasone.
SPECIAL PRECAUTIONS
With long-term use of the drug at high doses, systemic effects may occur. Patients receiving glucocorticosteroid therapy have a potentially reduced immune response; therefore, the risk of infection increases upon contact with patients suffering from infectious diseases (chickenpox, measles).
Intranasal glucocorticosteroids may lead to the development of glaucoma and cataract. Therefore, careful monitoring is required in patients with visual disturbances, increased intraocular pressure, or a history of cataract.
Glucocorticosteroids may cause growth retardation in children; regular growth monitoring is required.
Beclomil should be used with particular caution and under medical supervision in patients with adrenal insufficiency.
Infectious and inflammatory diseases of the nasal cavity and paranasal sinuses are not a contraindication for beclomethasone when appropriate therapy is prescribed.
