COMPOSITION

In 1 tablet contains:

Secnidazole - 1.0 g. 

INDICATIONS FOR USE 

·trichomonal urethritis and vaginitis (caused by Trichomonas vaginalis); 

·bacterial vaginosis; 

·intestinal amoebiasis (caused by Entamoeba histolytica); 

·hepatic amoebiasis (caused by Entamoeba histolytica); 

·  giardiasis (caused by Giardia lamblia)

DOSAGE AND ADMINISTRATION

Secnidox is taken orally immediately before meals, with a small amount of water.

Adult doses:

Trichomonal urethritis and vaginitis, bacterial vaginosis: 2.0 g as a single dose.

Intestinal amoebiasis:

    • acute symptomatic amoebiasis — 2.0 g single dose; 
    • asymptomatic amoebiasis, including focal and cystic forms — 2.0 g once daily for 3 days. 

Hepatic amoebiasis: 1.5 g daily as a single or divided dose for 5 days.

Pediatric doses:

Intestinal amoebiasis:

    • acute symptomatic amoebiasis — 30 mg/kg body weight as a single dose; 
    • asymptomatic amoebiasis, including focal and cystic forms — 30 mg/kg body weight once daily (single or divided doses) for 3 days. 

Hepatic amoebiasis: 30 mg/kg body weight daily as a single or divided dose for 5 days.

Giardiasis: 30 mg/kg body weight as a single dose.

PHARMACOLOGICAL PROPERTIES

Secnidox is an antiprotozoal drug of the nitroimidazole group with antibacterial activity; it exhibits bactericidal effects against gram-positive and gram-negative anaerobic bacteria, and amoebicidal activity (both intestinal and extraintestinal).

Secnidox is particularly active against Trichomonas vaginalisEntamoeba histolytica, and Giardia lamblia. After entering the microbial cell, secnidazole is activated by reduction of its 5-nitro group and subsequently interacts with cellular DNA. This leads to disruption of its helical structure and strand breakage, inhibition of nucleotide synthesis, and cell death.

Pharmacokinetics

After oral administration, Secnidox is rapidly and completely absorbed from the gastrointestinal tract. Bioavailability is approximately 80%. Secnidazole crosses the blood-brain barrier and is excreted in breast milk. The elimination half-life is about 25 hours, allowing a simplified dosing regimen that improves patient convenience.

CONTRAINDICATIONS

·hypersensitivity to secnidazole or other imidazole derivatives; 

·organic diseases of the central nervous system (CNS); 

·  pregnancy and lactation period.

ADVERSE REACTIONS

Secnidox is generally well tolerated, but side effects may occur:

Gastrointestinal system: digestive disorders, nausea, stomach pain, metallic taste in the mouth, glossitis, stomatitis.

Less common:

Hematopoietic system: moderate reversible leukopenia.

Allergic reactions: urticaria.

DRUG INTERACTIONS

Secnidox enhances the effect of indirect anticoagulants (coumarin or indandione derivatives) when used concomitantly.

Concomitant use with disulfiram may cause paranoid reactions and psychosis. Combination with alcohol leads to disulfiram-like reactions (abdominal cramps, nausea, vomiting, headache, facial flushing).

Secnidox increases plasma lithium concentrations when used with lithium preparations.

The drug is not recommended to be combined with non-depolarizing muscle relaxants (e.g., vecuronium bromide).

When combined with amoxicillin, activity against Helicobacter pylori is increased (amoxicillin prevents the development of resistance).

SPECIAL PRECAUTIONS

Secnidox should not be prescribed to patients with a history of blood dyscrasias. During treatment, reversible neutropenia may occur. Leukocyte counts return to normal after discontinuation of therapy.

Alcohol consumption should be avoided during treatment.

Use in pregnancy and lactation

The drug is contraindicated during pregnancy and lactation.

Pediatric use

The drug is used in pediatric practice.

Effects on driving and operating machinery

Secnidox does not affect the ability to drive vehicles or perform tasks requiring a high level of psychomotor coordination.

The drug should be stored out of reach of children and not used after the expiration date.
