COMPOSITION

5 ml of oral suspension contains:

 nifuroxazide – 200 mg. 

INDICATIONS FOR USE 

Acute bacterial diarrhea occurring without deterioration of general condition, fever, or signs of intoxication (in addition to rehydration through oral intake of fluids and electrolytes or, if necessary, their intravenous administration). The volume of administered fluid should correspond to the severity of diarrhea, age, and clinical condition of the patient. 

DOSAGE AND ADMINISTRATION

The medicinal product is taken orally, regardless of food intake. Before use, the bottle should be shaken until a homogeneous suspension is obtained. If necessary, the suspension may be taken with water. A measuring cup included in the package is used for dosing.

Children aged 1 month to 6 months: 100 mg (2.5 ml of suspension) twice daily (200 mg per day).

Children aged 6 months to 2 years: 100 mg (2.5 ml of suspension) three times daily (300 mg per day).

Children aged 2 to 6 years: 200 mg (5 ml of suspension) three times daily (600 mg per day).

Children over 6 years and adults: 200 mg (5 ml of suspension) 3–4 times daily

 (600–800 mg per day).

Duration of treatment: not more than 7 days.

Elderly patients: No clinical studies indicate differences in treatment response between elderly and younger patients.

Hepatic and renal impairment: No clinical studies indicate differences in treatment response in patients with impaired hepatic or renal function compared to patients with normal function.

Children: The medicinal product is not used in children younger than 1 month.

PHARMACOLOGICAL PROPERTIES

Pharmacodynamics

 Nifuroxazide is a nitrofuran derivative and acts as a locally effective intestinal antiseptic. When used at recommended doses, it does not produce systemic or toxic effects.

It is assumed that nifuroxazide inhibits dehydrogenase activity, suppresses cellular respiration processes, the tricarboxylic acid cycle, and protein synthesis in microbial cells. It also reduces toxin production by microorganisms.

It is highly active against gram-negative bacteria such as Campylobacter jejuni, Escherichia coli, Salmonella spp., Shigella spp., Clostridium perfringens, and pathogenic vibrios (Vibrio cholerae, Vibrio parahaemolyticus), as well as gram-positive bacteria such as Staphylococcus spp.

Nifuroxazide does not affect the saprophytic intestinal flora.

Pharmacokinetics

 After oral administration, nifuroxazide is practically not absorbed from the gastrointestinal tract, resulting in a high concentration of the active substance in the intestine (approximately 99% of the administered dose remains in the intestine).

Metabolism occurs in the intestine; more than 20% is excreted unchanged. Nifuroxazide and its metabolites are eliminated in the feces.

CONTRAINDICATIONS

  • Hypersensitivity to nifuroxazide, nitrofuran derivatives, and/or any of the excipients of the medicinal product; 
  • pregnancy; 
  • prematurity and neonatal period up to 1 month of age.

ADVERSE REACTIONS

Maculopapular erythematous rash, eczema, acute generalized exanthematous pustulosis, photosensitivity reactions, allergic reactions including rash, urticaria, angioedema, anaphylactic shock, thrombocytopenia, hemolytic anemia, agranulocytosis, increased transaminase activity, and discoloration of urine.

DRUG INTERACTIONS

Concomitant use with medicinal products that depress the central nervous system or with drugs causing disulfiram-like reactions is not recommended.

SPECIAL PRECAUTIONS

In infectious diarrhea with clinical signs of invasive infection, antibacterial agents with good systemic distribution should be used.

Excipients:

 The medicinal product contains ethanol (less than 100 mg per dose); therefore, it is not recommended for use in patients with liver diseases, epilepsy, individuals with alcohol dependence, as well as during pregnancy and lactation.

The product contains 1 g of sucrose per dose. Patients with rare hereditary disorders of fructose intolerance, glucose-galactose malabsorption, or sucrase-isomaltase deficiency should not take this medicinal product.

Pregnancy:

 There are insufficient data regarding possible teratogenic and fetotoxic effects of nifuroxazide during pregnancy.

Lactation:

 Use in women during breastfeeding is possible provided that treatment is short-term.
