TOLKIMADO

COMPOSITION

1 ampoule (1 ml) contains:

• • tolperisone hydrochloride — 100 mg; 
  • lidocaine hydrochloride — 2.5 mg. 

INDICATIONS FOR USE 

• • emergency or long-term treatment of pathologically increased skeletal muscle tone associated with organic neurological disorders (pyramidal tract lesions, multiple sclerosis, cerebrovascular disorders, myelopathy, encephalomyelitis); 
  • treatment of muscle hypertonia and muscle spasms associated with musculoskeletal disorders (spondylosis, spondyloarthritis, cervical and lumbar syndromes, osteoarthritis of large joints); 
  • rehabilitation after orthopedic and trauma surgery; 
  • as part of combination therapy for obliterative vascular diseases (obliterative atherosclerosis, diabetic angiopathy, thromboangiitis obliterans, Raynaud's disease, diffuse scleroderma), and diseases caused by impaired vascular innervation (acrocyanosis, intermittent angioneurotic dysbasia). 

Pediatric use:

• • Little's disease and other encephalopathies accompanied by muscular dystonia. 

DOSAGE AND ADMINISTRATION

Tolkimado is intended for intramuscular or intravenous administration.

Adults: 100 mg intramuscularly twice daily or 100 mg by slow intravenous injection once daily.

Children:

3 months to 6 years: intramuscularly or by slow intravenous injection at 5 mg/kg/day;

7 to 14 years: intramuscularly or by slow intravenous injection at 2–4 mg/kg/day, divided into 3 doses.

PHARMACOLOGICAL PROPERTIES

Tolkimado is a combination of tolperisone hydrochloride and lidocaine hydrochloride.

Tolperisone is a centrally acting muscle relaxant. Its mechanism of action is not fully understood. It has a high affinity for nervous tissue, reaching the highest concentrations in the brainstem, spinal cord, and peripheral nervous system.

Its main effect is related to inhibition of spinal reflex pathways. Together with suppression of impulse conduction in descending pathways, this provides its therapeutic effect.

Structurally, tolperisone is similar to lidocaine. It has membrane-stabilizing properties, reduces the electrical excitability of motor neurons and primary afferent fibers, and dose-dependently blocks voltage-gated sodium channels, thereby reducing the amplitude and frequency of action potentials.

Tolperisone also inhibits voltage-gated calcium channels and may suppress neurotransmitter release. In addition, it exhibits weak α-adrenergic blocking and antimuscarinic activity.

Lidocaine hydrochloride provides a local anesthetic effect. When Tolkimado is administered at the recommended dosage, lidocaine does not produce systemic effects.

CONTRAINDICATIONS

·hypersensitivity to tolperisone or any of the excipients of the drug (including lidocaine); 

·myasthenia gravis; 

·children under 3 months of age; 

·  breastfeeding (lactation).

ADVERSE REACTIONS

Blood and lymphatic system:

• • very rare — anemia, lymphadenopathy. 

Immune system:

• • rare — hypersensitivity reactions, anaphylactic reactions; 
  • very rare — anaphylactic shock. 

Metabolism and nutrition:

• • uncommon — anorexia; 
  • very rare — polydipsia. 

Psychiatric disorders:

• • uncommon — insomnia, sleep disturbances; 
  • rare — decreased activity, depression. 

Nervous system:

• • uncommon — headache, dizziness, somnolence; 
  • rare — attention disturbance, tremor, seizures, hypesthesia, paresthesia, rigidity; 
  • very rare — confusion. 

Eye disorders:

• • rare — blurred vision. 

Ear and balance disorders:

• • rare — tinnitus, dizziness. 

Cardiac disorders:

• • rare — angina, tachycardia, palpitations; 
  • very rare — bradycardia. 

Vascular disorders:

• • uncommon — hypotension; 
  • rare — flushing. 

Respiratory system:

• • rare — dyspnea, epistaxis, tachypnea. 

Gastrointestinal tract:

• • uncommon — abdominal discomfort, diarrhea, dry mouth, dyspepsia, nausea; 
  • rare — epigastric pain, constipation, flatulence, vomiting. 

Hepatobiliary system:

• • rare — mild liver function disturbances. 

Skin:

• • rare — allergic dermatitis, hyperhidrosis, pruritus, urticaria, rash. 

Renal system:

• • rare — enuresis, proteinuria. 

Musculoskeletal system:

• • uncommon — muscle weakness, myalgia, limb pain; 
  • rare — limb discomfort; 
  • very rare — osteopenia.

DRUG INTERACTIONS

·May enhance the effect of cardiac glycosides (e.g., digoxin). 

·When combined with antiepileptic drugs (carbamazepine, diazepam, sodium valproate), their anticonvulsant effect may be potentiated. 

·  Possible additive effects with other CNS-active drugs, depending on clinical context.

SPECIAL PRECAUTIONS

·Due to the lack of sufficient clinical data, Tolkimado should not be used during pregnancy (especially during the first trimester), unless the expected benefit to the mother outweighs the potential risk to the fetus. 

·  As there are no data on the excretion of tolperisone into breast milk, the use of the drug is contraindicated during breastfeeding.
