COMPOSITION

 In 1 tablet contains:

 Spironolactone 25 mg. 

INDICATIONS FOR USE 

·primary hyperaldosteronism: for short preoperative treatment or long-term therapy in cases of inoperability or patient refusal of surgery; 

·oedematous syndrome in chronic heart failure (as monotherapy or in combination therapy); 

·nephrotic syndrome when treatment of the underlying disease, diuretics, and sodium restriction are ineffective; 

·essential hypertension (as part of combination therapy); 

·hypokalaemia when other treatments are not possible; 

·  diagnosis of primary hyperaldosteronism

DOSAGE AND ADMINISTRATION

Spilactone is administered orally. The dose is determined individually based on the degree of water-electrolyte imbalance and the patient’s hormonal status. The daily dose may be taken as a single dose or divided into several doses.

Primary hyperaldosteronism: after diagnosis — 100–400 mg/day as preoperative preparation.

 In inoperable cases, long-term treatment with the lowest effective individually titrated dose may be used. In such cases, the initial dose should be reduced every 14 days until the lowest effective dose is reached. During long-term therapy, combination with other diuretics is recommended to reduce adverse effects.

Edematous syndrome (heart failure, liver cirrhosis, nephrotic syndrome): usual starting dose in adults is 100 mg/day (25–200 mg/day), divided into two doses. At higher doses, combination with diuretics acting on the proximal renal tubules is recommended, while the dose of Spilactone remains unchanged.

Children are treated at 3 mg/kg body weight per day, either as a single dose or divided into two doses.

Essential hypertension: used in combination with other diuretics; usual starting dose is 50–100 mg/day in two divided doses. Treatment should continue for at least 2 weeks, as the maximal antihypertensive effect develops after this period.

Hypokalemia: 25–100 mg/day when oral potassium supplementation or other potassium-sparing measures are insufficient.

Diagnosis of primary hyperaldosteronism: 400 mg/day for 4 days in divided doses. An increase in serum potassium during treatment followed by a decrease after withdrawal may indicate primary hyperaldosteronism.

PHARMACOLOGICAL PROPERTIES

Spironolactone is a potassium-sparing diuretic with a moderate natriuretic effect. Its diuretic action is due to antagonism of the adrenal cortex hormone aldosterone. This antagonistic effect occurs in the distal renal tubules: the drug reduces potassium excretion and increases sodium and water excretion without disturbing electrolyte balance; it decreases titratable urinary acidity and reduces urea excretion. It prevents aldosterone binding to intracellular protein receptors.

The renal effect of spironolactone reaches its maximum about 7 hours after oral administration and lasts for at least 24 hours. Increased diuresis leads to a hypotensive effect, which is not constant. The diuretic effect appears within 2–5 days of treatment.

Pharmacokinetics

After oral administration, spironolactone is rapidly absorbed from the gastrointestinal tract. About 90% is bound to plasma proteins. Bioavailability is 92–99% and increases when taken with food. It is distributed in the liver, kidneys, adrenal glands, and gonads.

Spironolactone is rapidly metabolized in the liver. The main pharmacologically active metabolites are 7α-thiospironolactone, 7α-thiomethylspironolactone, and canrenone. The anti-aldosterone effect is mainly attributed to canrenone, which accounts for about 50% of all spironolactone metabolites. Peak plasma concentration of canrenone occurs 2–4 hours after administration, with an average elimination half-life of 12.5 hours.

The drug crosses the placenta and is excreted in breast milk. It is eliminated via urine and feces.

CONTRAINDICATIONS

·hypersensitivity to the components of the drug; 

·hyperkalemia; 

·hypercalcemia; 

·hyponatremia; 

·anuria; 

·severe chronic renal failure; 

·pregnancy; 

·  lactation period.

ADVERSE REACTIONS

Gynecomastia may develop during treatment with Spilactone. This reaction usually resolves after discontinuation of the drug. In rare cases, some breast enlargement may persist.

Possible effects include malaise, nausea, electrolyte imbalance, hyperkalemia, leg cramps, and increased drowsiness.

DRUG INTERACTIONS

Concomitant use with indomethacin, ACE inhibitors, and potassium preparations may lead to hyperkalemia. To avoid hyperkalemia, spironolactone should not be used simultaneously with amiloride or triamterene. Concomitant use with carbenoxolone reduces the therapeutic effect of spironolactone, as carbenoxolone causes sodium retention. Spironolactone reduces vascular sensitivity to norepinephrine, which should be considered during general and local anesthesia in patients receiving spironolactone. Acetylsalicylic acid decreases the diuretic effect of spironolactone.

SPECIAL PRECAUTIONS

Special caution is required in patients with conditions predisposing to acidosis and/or hyperkalemia.

In cases of reduced renal function and hyperkalemia, transient increases in blood urea nitrogen may occur. Reversible hyperchloremic metabolic acidosis is also possible. In patients with kidney or liver disease and in the elderly, monitoring of serum electrolytes and renal function is necessary.

Spilactone is contraindicated during pregnancy.

If the drug is used during lactation, breastfeeding should be discontinued, as spironolactone may be excreted in breast milk.

Pediatric use

Spilactone is approved for use in children from the first days of life.
