About the Product

COMPOSITION

Each tablet contains:

dronedarone — 400 mg

INDICATIONS FOR USE 

·atrial fibrillation or atrial flutter (current or with a history of the condition) to reduce the risk of hospitalization due to cardiovascular events; 

·  ventricular premature beats (ventricular extrasystoles)

DOSAGE AND ADMINISTRATION

Treatment may be initiated on an outpatient basis.

The medicinal product should be taken orally with meals (at breakfast and dinner), with half a glass of water.

Before starting treatment with Droneda, therapy with other antiarrhythmic agents (such as flecainide, propafenone, quinidine, disopyramide, dofetilide, amiodarone, and sotalol) should be discontinued.

The recommended dose for adults is one 400 mg tablet twice daily (one tablet with breakfast and one tablet with dinner).

The maximum recommended daily dose is 800 mg.

PHARMACOLOGICAL PROPERTIES

Dronedarone is a multichannel antiarrhythmic agent.

    • It inhibits potassium channels (Class III antiarrhythmic effect), thereby prolonging the cardiac action potential and myocardial refractory period. 
    • It inhibits sodium channels (Class I antiarrhythmic effect). 
    • It inhibits calcium channels (Class IV antiarrhythmic effect). 
    • It acts as a non-competitive antiadrenergic antagonist (Class II antiarrhythmic effect). 

Through these combined actions, dronedarone helps maintain sinus rhythm and reduces the risk of recurrence of atrial fibrillation.

CONTRAINDICATIONS

·hypersensitivity to dronedarone or any component of the medicinal product; 

·second- or third-degree atrioventricular block or sick sinus syndrome (unless a functioning pacemaker is present); 

·severe bradycardia; 

·QTc interval ≥ 500 ms

·chronic heart failure (NYHA Class III–IV); 

·concomitant use of ketoconazole, itraconazole, voriconazole, clarithromycin, or ritonavir; 

·concomitant use of phenothiazines, cisapride, bepridil, tricyclic antidepressants, terfenadine, certain oral macrolides, and Class I or Class III antiarrhythmic agents; 

·severe hepatic impairment; 

·severe renal impairment; 

·pregnancy; 

·breastfeeding; 

·  children and adolescents under 18 years of age.

ADVERSE REACTIONS

The most commonly reported adverse reactions include:

    • diarrhoea, nausea, vomiting, abdominal pain, dyspepsia; 
    • weakness and fatigue; 
    • bradycardia; 
    • QT interval prolongation; 
    • increased serum creatinine levels; 
    • elevated liver enzymes (ALT, AST); 
    • rash, pruritus, allergic reactions; 
    • hepatic impairment, including rare cases of severe liver injury; 
    • heart failure or worsening of existing heart failure; 
    • interstitial lung disease (very rare).

DRUG INTERACTIONS

·Class I and Class III antiarrhythmic agents (amiodarone, sotalol, flecainide, propafenone, quinidine, disopyramide, dofetilide) — concomitant use is contraindicated

·Strong CYP3A4 inhibitors (ketoconazole, itraconazole, voriconazole, clarithromycin, ritonavir) — increase dronedarone plasma concentrations; concomitant use is contraindicated. 

·Medicinal products that prolong the QT interval (phenothiazines, cisapride, bepridil, tricyclic antidepressants, terfenadine, and certain macrolides) — increase the risk of serious ventricular arrhythmias and are contraindicated. 

·Digoxin — dronedarone increases digoxin concentrations; clinical monitoring and dose adjustment may be required. 

·Warfarin and other anticoagulants — coagulation parameters should be monitored when treatment is initiated. 

·Statins metabolized by CYP3A4 (e.g., simvastatin) — plasma concentrations may increase, raising the risk of adverse effects. 

·  Grapefruit juice increases dronedarone exposure and should be avoided during treatment.

SPECIAL PRECAUTIONS

·Treatment with other antiarrhythmic agents should be discontinued before initiating dronedarone. 

·ECG monitoring is recommended during treatment to assess cardiac rhythm and QT interval. 

·Do not use in patients with permanent atrial fibrillation, severe heart failure, or significant left ventricular systolic dysfunction. 

·Liver and renal function should be assessed before and periodically during treatment. 

·Treatment should be reassessed immediately if signs of hepatic injury, heart failure, or arrhythmia develop. 

·  Grapefruit juice should be avoided during treatment.
